If the double eyelid fold has come loose, do I need to have ptosis correction done again too?
This is the question most often asked by people who come in for consultations about revision double eyelid surgery after their fold has come loose.
To answer first: if the double eyelid fold has come loose, you do not necessarily need to repeat ptosis correction as well.
If the strength for opening the eyes is still maintained, you only need to redefine the line.
However, the situation differs depending on whether the previous surgery was done with an incisional or non-incisional method.
Today, I’ll organize how revision surgery differs in each case.

Incisional vs. non-incisional: the revision approach differs
The revision approach is completely different depending on the previous surgical method. Here is a simple summary.
| Category | If you had incisional surgery | If you had non-incisional surgery |
|---|
| Whether ptosis correction is maintained | Mostly maintained (strong adhesion) | High chance it loosened as well |
| Scope of revision | Double eyelid line only | Double eyelid + ptosis correction at the same time |
| Points to note | Need to check the fixation point | Proceed after removing the existing thread |
| Revision method | Incisional revision | Incisional or non-incisional, depending on the condition |

Why does a double eyelid fold loosen?
There are many reasons why a double eyelid fold loosens.
In most cases, the adhesion weakens because of the habit of rubbing the eyes frequently or due to aging,
but it can also happen when the epicanthic fold is strong or when the fixation was weak during the first surgery.
What matters most is distinguishing between “only the line has loosened” and “the strength for opening the eyes has weakened as well.”
The revision direction changes completely depending on this.
If you had incisional ptosis correction
If you had ptosis correction and double eyelid surgery together using the incisional method, in most cases the ptosis correction will still be maintained.
Because incisional surgery forms strong adhesion, the ability to open the eyes is often well preserved.
In this case, only the double eyelid line needs revision.
However, there is something to be careful about.
Surgical styles differ by clinic, and in some cases the fixation threads for ptosis correction and the double eyelid are tied together and operated on as one point.
In such cases, if the thread is loosened to revise the double eyelid, the ptosis correction that was otherwise fine may also come loose.
So when getting revision surgery at another clinic, you should definitely check whether there is a possibility that ptosis correction may loosen during double eyelid revision.
If you had non-incisional ptosis correction
The situation is a little different if you had the non-incisional method.
Because non-incisional surgery secures things with thread, the adhesion is weaker than with incisional surgery.
When the double eyelid fold loosens, there is a high chance that the ptosis correction has also become loose.
In such cases, the process usually goes like this.
- Remove the remaining thread from the previous surgery
- Revise the loosened double eyelid + ptosis correction at the same time
If adhesions have not progressed much, it can be done again with a non-incisional method.
If adhesions have formed significantly, revision surgery is performed with an incisional method.

When is the best time to get revision surgery?
You do not need to rush just because it seems like the double eyelid fold has loosened.
We recommend considering revision surgery after at least 6 months have passed.
The best results come once the scar tissue at the surgical site has softened and the swelling has completely gone down.
If you rush into revision surgery, the result may actually be worse.
Frequently Asked Questions (FAQ)
Q. If the double eyelid fold loosens, do I always need to repeat ptosis correction too?
No.
If the strength for opening the eyes is well maintained, you only need to revise the double eyelid.
It can vary depending on the previous surgical method, so an accurate diagnosis is necessary.
Q. I had surgery at another clinic. Can I still have revision surgery here?
Yes, that is possible.
If you tell us whether the previous surgery was incisional or non-incisional and when it was done, we can make a more accurate diagnosis.
Q. If I had the non-incisional method, can revision surgery also be done non-incisionally?
If there is little adhesion, it can be done non-incisionally again.
However, if adhesions have formed significantly, the incisional method may provide more stable results, so the condition needs to be checked in person.
To summarize,
Do not worry too much just because the double eyelid fold has loosened.
If you had incisional surgery → in most cases, ptosis correction is maintained, and only the double eyelid needs revision
If you had non-incisional surgery → after checking the condition, decide whether simultaneous revision is needed
Timing → after at least 6 months
The key is to accurately understand the current condition of the eyes.
Rather than a place that recommends “doing everything over again,” we suggest consulting a clinic that recommends only what is truly necessary.
