Can jawline reduction surgery be performed even if you have a special disease?
Can facial contouring surgery be performed even if the bone grows abnormally large?
Do you remember the story of the CRMO patient with an immune disorder that I introduced last time?
At Mooi Plastic Surgery, where unique beauty begins,
we’ll finally reveal the actual surgery process and results.
*If you haven’t seen Part 1 yet, please read it first—it will be much easier to understand :)
What we confirmed on CT before surgery

CRMO jawline reduction before-and-after front comparison, Mooi Plastic Surgery – mandibular enlargement
Before surgery, we carefully checked the nerve position on the CT images first.
As the bone had grown abnormally, bone had formed even around the area near the nerve.
We planned the surgery in a way that would preserve the nerve, and fortunately, the nerve position was not in a place that made surgery impossible.
What was different about this patient’s bone compared with that of a typical person?
| Comparison item | Typical adult | CRMO patient |
|---|
| Cortical bone thickness (outer bone layer) | 3–4 mm | Abnormally thick |
| Total bone thickness | About 15 mm | About 28 mm (roughly twice the average) |
| Internal bone structure | Relatively uniform | Uneven, with scar tissue mixed in |
| Cutting instrument | Standard surgical saw | A custom-made saw was needed |
On the CT images, this patient’s bone was noticeably different from that of typical people.
Repeated bone inflammation had left scars in the bone as well.
Isn’t it surprising that even bone can develop scar tissue?
Because the bone was nearly twice as thick as that of a typical adult,
a custom saw was prepared separately, since it seemed difficult to remove with the existing surgical tools.
How was the actual surgery performed?
Contrary to what was expected, there was not much bleeding during surgery. However, the surgery was by no means easy.
The reason was that the bone was so large that there was very little room for the surgical instruments to fit in.
We had no choice but to hold the bone while shaving it down,
but the bone became thin too easily, which created major limitations in the procedure.
The asymmetry was also severe.
Because the right side was larger, we removed as much as possible from the right side,
and adjusted the left side to match the symmetry, balancing both sides.
From below, you can clearly feel the difference in how much smaller the overall shape became.

What were the results?
The three risks that were concerning before surgery ultimately did not occur.
| Concerned risk | Actual result |
|---|
| Risk of bleeding | Less bleeding than expected, and the procedure was completed safely |
| Possibility of bone collapse | Did not occur |
| CRMO symptom flare-up | Inflammatory markers remained within the normal range after surgery |
The patient left with a bright expression, satisfied with the results.
Afterward, the patient planned to continue managing CRMO at the pediatric clinic they had already been visiting.

The meaning of this case
This surgery is the world’s first case of attempting surgical resection in a patient with excessive facial bone growth caused by CRMO.
If you are struggling because your jaw or face has become abnormally large for reasons other than the usual causes,
don’t worry about it alone.
We recommend first checking what may be possible through a consultation.
Frequently Asked Questions (FAQ)
Q. Is facial contouring surgery possible even when the bone is abnormally large?
The approach may differ from that used in typical cases.
Detailed CT analysis and the medical team’s experience are key. Please check whether it is possible through a consultation first.
Q. If I have an autoimmune disease, can’t I have surgery?
That is not always the case. It depends on the condition, whether the medication is being controlled, and the type of surgery.
Collaboration with the relevant department and medical staff is important, so please start with a consultation.
Q. Is there no possibility of recurrence after jawline reduction surgery?
In typical jawline reduction surgery cases, the chance of recurrence is low.
However, if there is an underlying condition such as CRMO, the disease itself also needs to be managed alongside surgery.
In summary,
Even in cases with a special condition such as CRMO,
surgical treatment may be possible with thorough preoperative analysis, collaboration with related departments, and an experienced medical team.
Don’t give up. Please start with a consultation first :)
