Revision of Lateral Canthoplasty - Fixation Method, Scar Correction, Recovery, and More

The inner and outer corners of the eyes are called, in Chinese characters, the medial canthus and lateral canthus. Medial canthoplasty is the front corner surgery that corrects the Mongolian fold, and lateral canthoplasty is the back corner surgery that extends or corrects the outer corner of the eye.

Lateral canthoplasty has been performed for quite a long time. There are many surgical methods for reshaping the lateral canthus when it is congenitally small or when it becomes drooped later in life. Double eyelid surgery and front corner surgery were also developed by drawing on methods used to correct congenital entropion and the medial epicanthal fold.

In the early days of back corner surgery performed as an extension of lateral canthoplasty, it was simply a simple back corner procedure that brought the inner conjunctiva toward the skin side to make the eyes appear larger. As a result, there were various problems such as no noticeable difference before and after surgery, inversion of the inner conjunctiva leading to ectropion, and significant back corner scars. Of course, no cosmetic surgery method is perfect, so some problems can occur to a certain extent. However, simple back corner surgery has little effect and often causes problems, so the newly devised back-corner lowering method is now drawing attention.

If simple back corner surgery has limitations in correcting the eye corner because it simply connects the inner conjunctiva and outer skin without new repositioning of deep tissue, back-corner lowering surgery can show definite correction of the eye corner through deep tissue repositioning and fixation. Because part of the lateral canthal ligament is cut, repositioning of the eye corner is possible; however, since the entire deep lateral canthal ligament is not severed, one advantage is that there is no further deformity such as excessive drooping of the outer corner.

If the fixation method used in revision back corner surgery is the back-corner lowering method, scars left by the previous simple back corner surgery can also be neatly corrected. Revision back corner surgery through back-corner lowering is different from back-corner restoration in that it does not make the eyes smaller while also removing scars.

Revision back corner surgery must be performed carefully because there may already be scars from the previous back corner surgery, or there may have been no effect. Careful work is essential, including identifying the problems with the previous back-corner design, creating a new and effective design, removing various adhesions and scars around the lateral canthus, and fixing the eye corner.

The recovery course after such meticulous revision back corner surgery may, of course, not be simple, but considering the effect of a more attractive lateral canthus and eye corner correction on the overall facial image, it is something that can be endured with enough patience.
