[Lari Plastic Surgery] Cases Where Revision Double-Eyelid Surgery Improves Scleral Show

Most people think that thick double eyelids make the eyes look bigger,

but in reality, that is not the case at all.

Of course, some people with large, prominent double eyelids do have big eyes,

but in general, people with thick double eyelids

have difficulty opening their eyes, so their eyes are not as large as expected.

When the double eyelids are thick, the area where the muscle and skin that help open the eyes adhere becomes wider,

which increases the load (the amount of work the muscle must do per unit area) and interferes with the force used to open the eyes.

That is why it becomes difficult to open the eyes wide.

As a result, when a person tries to open the eyes wider with effort,

the upper eyelid lifting muscle is not the only structure involved;

as a counteraction, the capsulopalpebral fascia on the lower eyelid is also pulled and strained,

causing the lower eyelid to move downward and resulting in scleral show.

Looking at the thickness of the double-eyelid line before surgery,

the patient’s right eye (left in the photo) is noticeably thicker than the left eye (right in the photo).

In this context, due to the asymmetry in double-eyelid thickness,

the right eye had slight scleral show, while the left eye did not.

This kind of phenomenon is improved when a thick double eyelid is corrected,

because the cause in the logic above is resolved, and the excessive scleral show is improved.

Through revision double-eyelid surgery, the uneven double eyelids were made more similar,

and after correcting the thick eyelid to a thinner one,

the scleral show in the right eye also improved.

If surgery is performed with a proper understanding of the structure, principles, and function of the muscles,

then with revision double-eyelid surgery alone, without many unnecessary additional procedures,

it is possible to comprehensively improve not only a sausage-like eyelid line and asymmetry,

but also asymmetric scleral show.
