Double eyelid surgery has been a plastic surgery procedure performed for a very long time.

When we hear from patients who come to the hospital for consultation about their eye surgery history,

it is not just surgery from 5 or 6 years ago,

but sometimes we find a history of double eyelid surgery from 20 or even 30 years ago.

Usually, when we look at the condition of the double eyelids of patients with such a long-ago surgical history,

we often see cases where a thick sausage-like double eyelid has been deeply pinched,

the incision scar is very severe,

the surgically created double eyelid has loosened, creating multiple folds above and below,

or the skin has sagged, making the line irregular and not smooth,

and other unfavorable forms.

This patient had a surgically created double eyelid that had loosened, and as the skin sagged,

multiple layered folds formed above and below the existing line.

To improve the complication of the double eyelid loosening, we performed surgery to remove the double eyelid.

If we created a double eyelid again, there would be a chance that it would loosen once more,

and if the double eyelid loosened, multiple folds would form as they do now,

making the eye shape look uneven.

By removing the double eyelid entirely and no longer leaving room for it to loosen,

a clean monolid eye shape was achieved.

There was also a sunken area between the brow bone and the eye, so fat grafting was performed.

Sunken eyelids can make a person look tired,

and in severe cases, they can become a factor that creates a double eyelid line as a third fold.

That is why, to improve these two issues, it is good to fill the sunken area of the eyelid.

The sunken area has been filled in well without becoming too excessive.

The fat grafted into the eyelid at this time was taken from the fat under the eyes.

If you look at the pre-surgery photo, the area under the eyes is bulging outward.

The bulge below is the fat surrounding the eyes,

and as people age, this fat sags downward and protrudes,

making it look like dark circles under the eyes or creating wrinkles like tear troughs,

so many people have it removed and repositioned.

So the fat removed from under the eyes was used to fill the sunken eyelid.

Even if it is not fat from this area, fat has to be taken from somewhere in order to fill the sunken area. If there is a lot of fat under the eyes, it is possible to achieve two goals with one surgery using that fat, and if there is no fat under the eyes, fat extracted from the abdomen or thighs can also be used for the grafting procedure.

Lower eyelid fat repositioning surgery is performed through an incision on the inner conjunctiva of the lower eyelid, so the incision line is not visible from the outside. However, even though this is a surgery to remove the double eyelid, when the eyes are closed, the incision scar can still be visible like this. In order to release the adhesions that form the double eyelid, the inner tissue of the eyelid must be carefully dissected, and because fat is also being grafted into the sunken eyelid, it is difficult to perform the surgery without an incision. Still, when the eyes are open, only the clean eye shape is visible, not the incision line, so there is nothing in particular to worry about.^^
