To be satisfied with canthoplasty,

the eyes need to open as much as desired, in the desired direction, and into the desired shape.

So, in many cases, patients get a cautious canthoplasty at clinics that perform simple canthoplasty with a short recovery period and a low price,

only to be dissatisfied and later visit our clinic for revision surgery.

The kind of satisfying opening that patients want usually means a clear widening effect and a gently lowered outer corner of the eye. At our clinic, canthoplasty clearly changes both the degree to which the eyes enlarge and the position of the outer corners.

The patient above had previously undergone fat grafting to the aegyo-sal area before surgery.

To improve the completeness of canthoplasty, not only the outer corner of the eye but also the area connected to the outer corner requires a complex effect.

If filler, grafted fat, or a foreign material in the aegyo-sal area is supporting the skin under the eye like a tube,

even if we try to lower the outer corner with canthoplasty, there may be a limit to how much it can be lowered, and after surgery, various effects may occur, such as deformation of the aegyo-sal shape or the lowered outer corner rising again.

Therefore, when performing canthoplasty to lower the outer corner,

it is important to dissolve aegyo-sal filler and scrape out and remove grafted foreign material or fat during surgery in order to improve the completeness of the canthoplasty.

When we lower the outer corner of the eye by hand, the double eyelid line behind the upper eyelid follows downward, so some people think that undergoing canthoplasty to lower the outer corner will affect the double eyelid line.

However, there is a clear difference between simulating it by hand and the actual surgical result, so, as you can see in the photo above, the result appears without any significant change to the double eyelid line.

And one thing to notice in the right eye in the photo above is this.

• Entropion surgery

Entropion surgery is a procedure that turns inward-curling eyelashes away from the eyeball to the outside. This surgery is performed when eyelashes naturally curl inward toward the eyeball and poke or irritate the eye, or when, after canthoplasty, the eyelashes curl inward and poke or irritate the eye.

Before the revision canthoplasty at our clinic, the lower eyelashes of the right eye were already curling toward the eyeball and poking it, so the patient underwent entropion surgery together with canthoplasty in order to improve the eyelash irritation before surgery and prevent it after surgery.

Looking at the before-and-after photos, the lower eyelashes that had been standing up and touching the eyeball were not worsened by canthoplasty and were well corrected with entropion surgery.

Entropion surgery has a high chance of recurrence, so consistent care is needed, including avoiding rubbing the eyes frequently or forcefully.
