[Lari Plastic Surgery] Bulging Eyes? Sunken Eyes? Smoother with Fat Grafting and Double Eyelid Removal

Depending on relative effects,

if the person next to me is slim, I appear relatively heavier,

and if the person next to me is short, my height appears relatively taller.

This also applies within one person:

if both the upper and lower body are slim, but the upper body is slimmer,

the lower body may appear relatively fuller and more prominent.

The same is true for the eyes. I may not have bulging eyes,

but because of hollowing or a sunken appearance around the eyelids near the eyeballs,

the eyeballs can appear to protrude relatively, making the eyes look bulging.

Just like this patient.

Looking at the condition before surgery, the upper eyelids are severely hollowed,

drawing inward so much that a crease forms just below the eyebrows.

Because of this, the eyeballs look as if they protrude more,

and the area around the eyes shows an overall uneven contour.

This appearance can make a person look tired and worn out.

So when the hollowing is this severe, fat grafting is performed on the upper eyelids

to ease and improve this effect.

Even if the upper eyelids look sunken before surgery, it does not mean there is no fat in the eyelids.

The 'orbital fat' that usually comes out during double eyelid surgery is present in sufficient amount in this patient as well. However, the visible hollowing here is not because of the orbital fat surrounding the eyeball, but because there is no 'septal fat' on the outer side of the septum that surrounds the orbital fat, right under the skin.

Because orbital fat and septal fat are located in different structural positions around the eye,

in order to improve eyelid hollowness, fat must be grafted into the area where septal fat should be,

so that the hollowing is fully corrected, as seen in the postoperative result.

First, the adhesions in the sunken area must be released, and the outer layer rather than the inner side of the septum must be carefully dissected. Next, an adequate amount of fat for grafting is harvested,

and finally, the appropriate amount of fat to improve the degree of hollowness is placed in the exact location,

then the layer is closed and lightly fixed so that the fat does not move excessively inward.

In this way, the eyelid hollowing was well corrected.

This is the postoperative result after performing fat grafting on the upper eyelids while also removing the bothersome double eyelid crease.

The visible double eyelid crease, though not thick, was removed well,

and the sunken upper eyelid became smooth without uneven contours.

It may seem like many things were done in the surgery to obtain the before-and-after photos above,

but in the end, the only visible scar is the incision line seen when the eyes are closed.

The fat grafted to the upper eyelids is harvested from excess orbital fat through the inside of the lower eyelid conjunctiva,

so there is no external scar.

Also, because some fat is removed from beneath the eyes,

if there is a bit of protruding fat under the eyes, this is also partially corrected.

In addition, rather than making separate incisions for removing the double eyelid crease and for fat grafting,

a single incision is used to remove the double eyelid crease and to dissect down to the layer needed for fat grafting,

so the scar remains neatly as just one line.
