[Rari Plastic Surgery] Correcting Ptosis Side Effects While Also Removing a Double Eyelid

I had double eyelid surgery, and my eyes became uneven.

I had eye-opening correction surgery, and my eyes became uneven.

These two sentences have very different outcomes.

Saying that double eyelid surgery made the eyes uneven usually means:

1. the thickness of the double eyelid line is different, or
2. the shape of the double eyelid line is different, or
3. the type of double eyelid line is different.

However, saying that eye-opening correction surgery made the eyes uneven means

that “the eyes open differently.”

This patient had previously undergone non-incisional eye-opening correction at a clinic that most people would recognize just from the name.

We cannot know what the eyes were like before the eye-opening correction, but after the non-incisional eye-opening correction surgery,

it appears that the degree of opening between the two eyes became significantly different.

Rather than an issue with the double eyelid line, the difference in how much the eyes open is severe, and the left eye, aside from the difference itself, is overcorrected to the point that the way it opens does not look good.

We call this symptom in the left eye lagophthalmos.

The exact definition of lagophthalmos refers to the eye not closing properly and completely like a rabbit’s eye, but in such cases, it is generally accompanied by a symptom in which the eyes open so widely that the pupils are fully visible. The preoperative condition above is included in the same context.

The patient came to our clinic to have a double eyelid removal procedure because they thought this difference in the eyes was caused by the double eyelids. However, if you simply remove the double eyelids while the eye-opening correction remains in place,

the difference in how the eyes open will remain the same while only the double eyelids are removed.

So we proceeded with a double eyelid removal procedure + lagophthalmos correction surgery with a plan to release the overcorrection from the left eye-opening correction and align both eyes more evenly.

Through an incision, we released the adhesion from the left eye-opening correction to ease the overcorrection, and at the same time, we also released the double eyelid adhesion to remove the double eyelid, creating a naturally monolid look with both eyes opening similarly.

Not only was the double eyelid removed, but once the degree to which both eyes opened was adjusted,

it felt much more comfortable to look at the eyes than before surgery.

During the operation, we repeatedly had the patient open and close their eyes to check the result several times, so the patient may have found the surgery somewhat difficult. However, if too much of the carefully performed eye-opening correction is released, differences can appear again, so for the best result, we performed the surgery carefully and precisely even though it was tiring.

As with anything, balance is important,

and once the left-right balance was corrected, the result looked nice and stable.

Because the above surgery is an incisional procedure, it is impossible to have no scar like with non-incisional eye-opening correction. However, even though it was an incisional procedure, the incision line was made very close to the eyelashes,

and it was carefully sutured without misalignment of the skin layers so that the scar would not remain severe.

It is still in the recovery period, so the incision scar is still visibly red, but it will gradually improve over time.

Even after surgery, you can see that the eyes still close well.
