I’m looking at before-and-after photos of surgery to remove a natural double eyelid and create monolids.
Natural double eyelids are a bit different from surgically created double eyelids. Rather than forming as a single, neat line, they often appear as multiple folds, like wrinkles.

One reason these multiple folded double eyelids are often seen in natural double eyelids is excess skin.
Think of the folded bellows of an accordion—it may help make sense of it.
Just as the outer surface area that forms the folds in a bellows is wide,
when there is a lot of extra skin, folds are more likely to form.

These layered double eyelids look like wrinkles even when the eyes are closed,
and
when looking downward,
you can see thicker, darker folds.
Of course, after double eyelid removal surgery, these kinds of lines are
cleanly gone, as shown in the after-surgery photos.
What matters more than when the eyes are closed or looking downward is when the eyes are open.
A person’s eyes are open almost all the time except when sleeping.

This is the frontal view with the eyes open.
You can see that the double eyelids are completely gone and the eyes have become monolids, right!?
If the adhesions in the internal eye tissues that form the double eyelid are carefully released
and the surgery is performed to create complete monolids, the eyes can become smaller.
So, even if it is not Müller’s muscle fixation ptosis correction,
performing a slight ptosis correction with levator aponeurosis fixation can prevent the eyes from becoming too small while still allowing them to change into perfect monolids.

The monolids have been maintained for more than 2 years after cleanly removing the double eyelids.
Theoretically, there is a possibility of recurrence,
but if double eyelid removal surgery is performed with an incision rather than a non-incisional method,
the possibility of recurrence is significantly lower, allowing the monolids to be maintained for a long time.
