[Lali Plastic Surgery] Comparison Photos of the Before-and-After Differences of Epicanthoplasty Restoration, Revision Lateral Canthoplasty, and Fat Graft Removal Surgery (Under-Eye + Cheekbone Area)

This patient had previously undergone epicanthoplasty and lateral canthoplasty.

However, after the epicanthoplasty, the inner corner of the eye became too sharply pointed, and the scar became visible,

so the patient was considering epicanthoplasty restoration surgery.

Restoration is a procedure based on the concept of closing a previously opened area. If the opened area is simply closed, the eye length becomes shorter,

which raised concerns about whether the eyes might look smaller or more cramped after surgery.

To solve this concern, epicanthoplasty restoration surgery was performed together with revision lateral canthoplasty,

and the results were very good.^^

Before surgery, the patient had also undergone lateral canthoplasty at another clinic,

but the effect was so minimal that it was hard to tell whether the procedure had actually been done.

With this revision lateral canthoplasty, the patient was able to get the benefit of the lateral canthoplasty effect,

while also resolving the possible issues of smaller-looking eyes and a cramped appearance that can occur with epicanthoplasty restoration.

Our clinic’s lateral canthoplasty, which lowers the outer corner of the eye,

has a large range of eye widening, so there is a rumor that the eyes may not close properly after surgery.

But that rumor is just a rumor. Even if the eyes have become this much larger,

as shown in the photo, they still close very well.^^

And those with a sharp eye may have noticed that,

although it is not very noticeable, if you look closely at this after-surgery photo,

you can see an incision line along the lower eyelid area near the eyelashes.

This line is the incision made to remove the fat graft from the under-eye and cheekbone area,

so the incision extends downward beneath the eye, almost as if lower blepharoplasty had been performed.

The patient had previously received fat grafting to the under-eye and front cheekbone area.

Facial volume fat grafting is one of the contour-improving procedures often chosen by people with little facial fat.

When fat is grafted into areas lacking volume, the volume is restored,

making the face look more three-dimensional, younger, and more lively.

However, this kind of fat grafting must be performed carefully to maintain smooth lines;

otherwise, unnatural contours like this can form.

Looking at the before-and-after in motion makes it very clear that the epicanthoplasty restoration and revision lateral canthoplasty were done well,

and the oddly protruding area on the cheekbone area (the part marked with the yellow dot)

has become smooth and natural after surgery.

A photo alone may make it hard to tell, so I marked the area with arrows for clarity.

In the pre-surgery photo, the area indicated by the arrow looks slightly indented and contoured,

while in the post-surgery photo, the area indicated by the arrow forms a smooth curve.

To remove the fat graft placed in the cheekbone area like this, an incision must be made along the lower eyelid line,

and the skin and muscle layers must be carefully dissected without damaging the muscle, down to the target area,

where the fat cells that have settled between the muscle and tissue must be scraped out one by one and removed.

People undergo fat grafting surgery to achieve a smoother face, but it is something that should be thought through carefully beforehand.

If it created unwanted contours or unnaturalness in the face,

it may be worth considering fat graft removal surgery.
