[Lari Plastic Surgery] Correcting Uneven Eyes with Forehead Botox After Double Eyelid Removal Surgery

Someone with naturally uneven double eyelids of different sizes

was considering double eyelid removal surgery.

Before visiting our clinic, they had consulted many other hospitals as well, but trusted our clinic and entrusted us with the surgery.

Let’s look at the course of the patient’s treatment, including double eyelid removal surgery, the results, and even forehead Botox.

First, let’s look at the eyes.

There were noticeable natural double eyelids on both eyes, and the size of the double eyelids on the left eye (right side in the photo) was larger than that of the right eye (left side in the photo), and the eyes were opening well.

So, we proceeded with surgery to remove the large, thick natural double eyelids as much as possible, making them close to monolids.

The surgery was performed with an incision, including partial skin excision, loosening of the eyelid adhesion, and stitch removal on day 6.

This is the appearance at 3 months.

The surgery went well and the double eyelids were removed nicely, but the left eye (right side in the photo) remained a concern for the patient.

They wanted eyes that looked more masculine rather than round, but the left eye felt bigger and as if the line was still visible.

So we recommended forehead Botox. When you open your eyes, if you use not only the eye muscles but also the forehead, the eyes tend to open wider even if you do not intend them to.

However, we decided to observe a little longer, and the patient returned to the clinic 9 months after surgery.

Although the difference had eased somewhat compared to before, there was still asymmetry between the two eyes, so forehead Botox was performed.

The reason for the forehead Botox was the same as above. If you unconsciously use your forehead when opening your eyes, your eyes may appear larger, and because the overall muscles are not being used evenly, asymmetry can occur.

In fact, even during the pre-surgery consultation, we had recommended it because it seemed likely to be needed together with the surgery. However, since the patient was not considering Botox at the time, we could not insist on it.

Then, one week after the forehead Botox, the two eyes were well balanced and looked similar.

By suppressing the use of the forehead muscles with Botox, the eyes were opened only with the eye muscles, which reduced the difference in eye size.

Botox is injected directly into the muscles that need treatment, and its effects can appear as early as 2 to 3 days, or within a week at the latest.

Botox can help positively in correcting uneven eyes like this by suppressing muscle use, but for people who rely heavily on the forehead to open their eyes, forehead Botox may instead make the eyes feel heavy and harder to open, and they may feel as if their eyes have become smaller, so caution is needed.

In this way, even though we perform surgery while anticipating the results to a certain degree, there is no result that can be predicted 100%.

That is why we need to continuously follow the progress, observe the changing condition of the eyes, provide the necessary treatment according to the situation, and monitor the changes so that we can reach the best possible outcome.
