The first case is when the height of the double eyelids is different.

This refers to a state in which the eyes are similar in size, but the height and shape of the double eyelids differ, causing the thickness of the double-eyelid lines to be asymmetric. This usually happens when, after double-eyelid surgery loosens, a new line forms above or below the existing double-eyelid line.

In such cases, after consulting about the desired height, surgery is performed by creating a new line. The double-eyelid line heights that were different before surgery, and the multiple overlapping lines, are neatly organized after surgery into a single line at a similar height.

At this time, if a line is made too high or surgery is performed with a method that does not suit the eye condition, there is a high chance that the double eyelid will loosen again and become uneven like this. Therefore, the current condition must be accurately identified, and the surgery must be performed with an appropriate method at the proper height.

When correcting asymmetry in the double-eyelid line and there is insufficient eye-opening strength, the eye-opening strength can also be corrected at the same time to enlarge the eyes and achieve a better result.

The second case is when the eye size (the degree of pupil exposure) is different.

The shape of the double-eyelid lines is roughly similar, but the size of the pupils is clearly different on both sides. A difference in the visible size of the pupils usually means there is a difference in eye-opening strength. Of course, in addition to that, there can also be several other reasons, such as the amount of excess eyelid skin and sagging.

So in this condition, the main procedure is eye correction to adjust the eye-opening strength on both sides, and, as a supplementary step to matching both sides, skin removal or line correction is also performed so that the degree of pupil exposure is made similar. The third case is when both the double-eyelid line size and the eye size are different.

Looking at the yellow arrow area, the thickness of the double-eyelid line differs, and on the right side of the photo, the double-eyelid line is thicker. You might think the eye sizes are roughly similar, but when looking at the number of reflections from the lights in both eyes within the red circle in the preoperative photo, you can see that the left eye in the photo is opened wider.

Compared with the first and second cases above, although each difference is smaller, there are still differences in both, so in this case, a combination of eye correction surgery to adjust eye-opening strength and revision double-eyelid surgery to match the thickness of the double-eyelid line is needed.

If you look at the postoperative photo, unlike before surgery, the thickness of the double-eyelid lines has been matched, and you can also confirm that the number of light reflections seen in the pupils within the red circles has changed to two on both sides.

At a glance, both eyes have been corrected in a complex but similar way.

The last case is when the double eyelids that can become asymmetrical are completely removed.

As mentioned above, even if one corrects one way in this situation and another way in that situation, if the double eyelids loosen or if the eye correction loosens, they can become uneven again.

When you think about the different factors involved, completely removing the double eyelids may be the most fundamental way to escape the cycle of asymmetrical unevenness.

Before surgery, the eyes had natural double eyelids that were uneven. After surgery, once the double eyelids were completely removed, both the asymmetry and the double eyelids disappeared, producing a good result that fully resolved the concern.

In principle, it is impossible to completely eliminate the asymmetry a person naturally has, so even with double-eyelid removal surgery, there may still be slight differences. However, when it comes to asymmetry caused by surgery or the limitations of correction, this can be the most effective method.

There are various reasons why both eyes can become asymmetrical. In addition to differences in double-eyelid line size and eye-opening strength, there can also be differences in the depth of fixation of the double eyelids, which can affect the degree of eyelash lifting, differences in the degree of sagging of the eyelid skin or upper eyelid skin, and various other causes such as unconscious use of the forehead muscles.
