To naturally remove the curve in the middle of the bridge like this,
hump nose correction surgery is performed.
Hump nose correction surgery is not very different from general rhinoplasty,
but if there is one difference, it is this: 'osteotomy.'
Osteotomy is the key point that increases the completeness of hump nose surgery.

Osteotomy means cutting and shaping the protruding part of the bridge, the hump.
Because the word osteotomy can feel like it means cutting bone,
you may think that the nasal bone is shaved down that much,
but the nasal bone is smaller than most people think and occupies only part of the nose.
So during osteotomy,
the nasal bone, called the nasal bone, and part of the lateral nasal cartilage that makes up the nose are cut together.

The photo above shows the hump reshaped by osteotomy (nasal bone + nasal cartilage).
It has the shape of a long, spindle-like hump that has been cut out.
It was cleanly cut without unnecessary excess, and as shown below, a good result was achieved.

Once the osteotomy is done neatly, we can see a smooth bridge.
The reason osteotomy is the key point in hump nose correction surgery
lies in the nature of osteotomy itself.
As explained earlier, osteotomy is the cutting of bone.
Once the bone has been cut, it cannot be reattached exactly as before.
Therefore, it is necessary to precisely design the bone and cartilage to be cut and to remove them without error.
An osteotomy that is too minimal leaves the hump behind and cannot create a smooth bridge,
while an excessive osteotomy can remove more than just the hump and, in severe cases, create a depression in the bridge.

A hump nose that requires osteotomy has a pronounced curve but is fundamentally high at the bridge,
so if the osteotomy is done naturally and well,
a beautiful bridge line can be created without placing a separate implant.
At this time, it is important to properly lift the drooping nasal tip to complete the nose shape,
and the nasal tip is also operated on using autologous tissue rather than an implant (ear cartilage, septal cartilage, rib cartilage, etc.),
maximizing the effect and minimizing side effects.
If the bridge and the nasal tip do not harmonize with each other, a satisfying result cannot be achieved.

Raising the nasal tip does not end with the tip itself;
for a prettier nose line, the angle formed by the tip and the upper lip must also be widened appropriately.
The angle formed by the nasal tip and the upper lip is called the nasolabial angle,
and the ideal angle is about 100° to 110° for women and 90° to 95° for men.
Before surgery, the nasal tip had drooped and the nasolabial angle was less than 90°,
but as it was widened to an ideal degree, the completeness of the nose line improved much more.
Looking at recovery after hump correction surgery,
when osteotomy is performed, the area where the bone has been shaved remains unstable until it settles,
so a splint must be worn for about one week.
Because of the splint, you cannot immediately check the shape and line of the nose,
but after about 1 to 2 weeks, the splint is removed and the major swelling goes down,
allowing you to see the nose shape you wanted.
After the splint and stitches are removed, daily life is fully possible,
but for a while, care should be taken not to apply irritation or impact to the nose.

Even a hump that does not look severe to the naked eye, to the point where you might think, 'This is not really a hump,'
if surgery is performed without addressing the slight curve at the base,
the curve can still remain after surgery, making it difficult to create a smooth line.
In such cases, if the hump is not severe, another hump correction is performed by refining it with a rasp instrument instead of cutting the bone with osteotomy.
For this patient as well, by raising the nasal tip and ideally adjusting the narrowed nasolabial angle,
the line from the bridge to the upper lip changed to look much more pleasing.
