Many people who are considering rhinoplasty often ask whether an implant needs to be inserted.
Whether or not an implant is used depends on the current condition of the nose and the desired nasal shape, but
if the surgery is for hump nose correction, it can be performed naturally without an implant.
In fact, many hump nose correction surgeries use the patient’s own tissue without implants such as silicone,
and especially in a first surgery, septal cartilage is used to achieve good results.

Hump nose correction surgery first involves dissecting the tissue inside the nose, then removing the hump on the protruding bridge.
During hump correction, if the hump is shaved down too much due to insufficient skill or a lack of understanding of the nose’s anatomical structure,
the previously protruding bridge can become hollowed out inward instead,
resulting in an artificial and unnatural nose shape with no bridge, so caution is needed.
When carving to remove the hump, the instruments used are rougher than expected, so
to remove the hump naturally without overdoing it,
subtle and delicate skill in using the instruments is essential.

After hump removal is completed, a central support is built to create a nose tip that connects naturally with the bridge.
The most commonly used material at this stage is septal cartilage.
Septal cartilage is the cartilage found within the wall between the two nostrils,
and it is flatter and firmer than ear cartilage, but softer than rib cartilage,
making it very suitable for use as a support in a first rhinoplasty with fresh tissue.
Of course, because the size and thickness of each patient’s septal cartilage differ,
even in a first surgery it may be difficult to use, and in some cases rib cartilage is used.
In revision surgery where septal cartilage was used before, or
if the patient wants a higher and more refined nose tip, rib cartilage is also used.

Once the support is built to the desired height,
a process is needed to naturally connect the nose tip and the bridge.
At this stage, the nose tip is covered with cartilage to smooth it out,
and cartilage is carved and placed into any empty space so that the bridge and tip connect naturally.
If empty space beneath the skin is left unfilled, dents can form in an area that is not structurally firm,
so it is important to finish the surgery properly through this process.
After hump nose correction surgery, the most noticeable changes are the three-quarter view and the side view.

Because the hump shape is more noticeable from the side than from the front, the corrected appearance is also clearly seen.
When looking at people with hump noses, it is common for the bridge to protrude and the tip to drop, making the nose appear longer.
If the midface also looks longer because of the long nose, the face may appear older than it actually is,
so when hump nose correction shortens the nose length, the change toward a more youthful image is very significant.

Because of the drooping nose tip, the nostrils were not easily visible from the front before surgery,
but when the drooping tip is lifted while correcting the hump, the nose becomes shorter,
so the nostrils may be more exposed after surgery than before.
However, the degree of tip rotation is not excessive and remains natural, so it can actually look better.

Using an implant such as silicone makes it easier to shape the nose,
but there are long-term risks such as a foreign-body sensation or inflammation.
In contrast, autologous septal cartilage comes from the patient’s own body, so there is no rejection reaction,
and natural, stable results can be expected.
Hump nose correction is a precise surgical procedure that goes beyond simply shaving down a protruding area.
Bridge line + nose tip support + functional stability
Good results require surgery that balances all three of these factors.
That is why, before surgery, it is important to have a thorough consultation based on an analysis of your cartilage volume, skin thickness, facial骨 structure, and other factors.
