[Lari Plastic Surgery] Cases in Which Three-White Eyes Improve Through Revision Double-Eyelid Surgery

Most people think that thick double eyelids create large eyes,

but in reality, that is not the case at all.

Some people with large, dramatic double eyelids do of course have large eyes,

but people with thick double eyelids usually have difficulty opening their eyes, so their eyes are not as large as they might expect.

When the double-eyelid crease is thick, the area of adhesion between the muscle that opens the eye and the skin is extensive.

This increases the loading (the amount of work the muscle must perform per unit area), which interferes with the force used to open the eye.

That is why it becomes difficult to open the eyes wide.

As a result, when a person exerts force to open the eyes wide, not only the upper-eyelid opening muscle but also the lower-eyelid capsulopalpebral fascia is pulled and placed under tension through its counteraction.

This causes the lower eyelid to move downward, resulting in a three-white eye appearance.

Looking at the thickness of the double-eyelid crease before surgery,

you can see that the patient's right eye (the left side of the photo) had a thicker crease than the left eye (the right side of the photo).

In this context, the asymmetry in crease thickness meant that there was a slight three-white eye appearance in the right eye, while there was none in the left eye.

This same phenomenon can be improved by correcting the thick double-eyelid crease,

because the cause of the above mechanism is resolved, improving the previously excessive three-white eye appearance.

Revision double-eyelid surgery was used to make the uneven double-eyelid creases more similar and to thin the previously thick crease.

As a result, the three-white eye appearance that had been present in the right eye also improved.

When surgery is performed after properly understanding the structure, principles, and role of the muscles,

revision double-eyelid surgery alone can improve not only sausage lines and unevenness but also asymmetrical three-white eyes,

without the need for numerous unnecessary additional procedures.
