[LARI Plastic Surgery] Structural Line Rhinoplasty Revision Connecting the Nasal Bridge, Dorsum, and Tip

When you first had nose surgery, you were clearly satisfied, but at some point as time passed, you may have started to feel that

'Your nasal tip seems to have dropped a little compared to before,'

'Your nose looks longer overall,'

'Your nasal bridge and tip do not seem to flow together smoothly.'

More people than you might expect experience these kinds of concerns.

As a result, when consulting about revision rhinoplasty, some people are not simply concerned about the height or shape of the nasal tip. Instead, they consider surgery again because of changes in the overall nasal line over time.

In the preoperative photo on the left, you might wonder whether surgery is really necessary,

but in the postoperative photo on the right, the line appears somehow smoother.

The difference is so subtle that it may be difficult for someone who is not particularly perceptive to identify what has changed,

but the overall impression is clearly different when you look at the shape of the nose.

From the side, the changes are even more apparent.

The key point is the nasal dorsum line.

In the preoperative photo, the slightly dropped nasal tip over time after the previous nose surgery had created an upward-bulging curve in the nasal line.

It looked uneven, as though the nasal bridge were slightly bent.

The nose is not made up of separate areas that exist independently.

Because the nasal bridge -> dorsum -> tip -> columella are connected as one continuous line,

when correcting the position and shape of the nasal tip, the overall sense of continuity must also be considered.

In this patient’s case as well, the tip was not simply raised as high as possible without considering the design. Instead,

the surgery was performed while assessing the overall line extending from the nasal bridge to the tip,

resulting in a natural and smooth change to the previously bent-looking dorsum and nasal line.

As a result, it is not simply that the nasal tip was raised.

The entire nose appears more evenly refined and balanced.

Although this was a procedure that adjusted subtle differences, changes can also be seen from the front.

As the nasal tip was refined more softly than the original nose, not only did the overall shape of the nose change,

but the curve beneath the nose connecting to the nostrils also became more three-dimensional,

and the nostrils appeared slightly slimmer when comparing the before-and-after photos.

A separate alar reduction procedure was not performed, but as the nasal tip was raised, an effect that made the nostrils appear smaller was seen.

When the proportions of the entire nose become more balanced as the shape and position of the nasal tip change,

the proportions of the nose and nostrils seen from the front may also change.

Ultimately, nose surgery should not be performed by looking at only one area.

The overall impression, continuity, and lines visible from multiple angles must all be considered together.

Those considering revision rhinoplasty may be concerned not only about the shape of the nose,

but also about scarring on the columella.

It is concerning enough to worry about significant scarring,

and it cannot be ruled out that additional incisions could result in two or even three scars.

Fortunately, this patient’s skin and tissue had recovered relatively well,

and as shown in the postoperative photo, the columellar incision healed with the scar barely noticeable.

However, the degree of scarring varies from person to person depending on various factors, including skin type and tissue condition, such as the number of revision procedures.

Therefore, not everyone can achieve the same result.

For this reason, in revision surgery, it is important not only to consider the design but also to carefully assess the current condition of the tissue, skin condition, and other factors before proceeding.

(Basically, there should be an interval of at least six months from the previous surgery!!)

Revision rhinoplasty requires consideration of many factors resulting from the previous surgery,

including the condition of the tissue, adhesions or scarring, and any materials remaining inside the nose and their condition.

Even a seemingly simple concern, such as a nasal tip that appears to have drooped, is not merely a matter of height.

The direction and angle of the tip, its connection to the nasal bridge, and the balance between the columella and nostrils are among the many factors that must be considered together during surgery.

Therefore, a sufficient consultation and careful decision-making are necessary.

Rather than simply bringing in a single photo and asking for your nose to be made that way,

it is important to understand the current condition of your nose, coordinate the direction of the procedure,

and work toward the best possible result within what can realistically be achieved.

Rather than making the nasal tip 'unconditionally high and sharply defined,'

this case was approached by correcting the areas that needed improvement while reducing anything that felt excessive, taking the existing condition of the nose into account.

The surgery was performed with the goal of creating a natural appearance,

which may have made it more difficult than completely creating the nose anew.

If, over time after nose surgery, your nasal tip appears to have dropped somewhat,

or you are concerned that your nasal bridge line looks bent compared to before,

rather than simply thinking, 'Do I need to raise it again?!,'
