Many patients mention that they have a peanut-shaped face and ask how it can be corrected.
What kind of face is a peanut-shaped face?


As shown in these photos, a face with prominent cheekbones, hollow temples and hollow cheeks, and a chin area that appears to bulge forward when viewed from the front is generally called a peanut-shaped face.
A peanut-shaped face may make the face look longer than it actually is, and it may also make you look older.
In such cases, it can be somewhat difficult to correct with just one surgery or with bone surgery alone.
This is because the face is a complex structure composed not only of bone, but also soft tissues such as fat and muscle.
So I will explain step by step how it can be addressed.
Prominent cheekbones can be corrected with cheekbone reduction surgery.
If only the side cheekbones protrude, surgery through a scalp incision, such as a 230-degree 3D cheekbone reduction, may be enough to correct it.
If the 45-degree cheekbones are very prominent, a 3D cheekbone reduction may help.

Hollow temples and hollow cheeks
These areas are difficult to correct 100% with cheekbone surgery alone.
I have written a column about correcting hollow cheeks, so I think it would be helpful to refer to that once.
Hollow temples and hollow cheeks can be dramatically improved through fat grafting.
Fat grafting is a procedure in which fat is harvested from areas such as the thighs or abdomen and then transplanted.
Usually, after two sessions, including a second round using frozen fat, about 80% of the desired volume can survive.
In this way, fat grafting can also greatly reduce a bumpy appearance.

If the area below the cheeks is bulging like a candy wrapper or has a lot of fat, improvement can be achieved through buccal fat removal and facial liposuction.

Buccal fat removal is a procedure in which an incision of about 0.5 cm is made inside the mouth to remove part of the deep fat pad.
Through this method, deep cheek fat can be removed, and if there is a lot of subcutaneous fat, it can be improved with facial liposuction.

If the actual square jawbone protrudes outward or the muscles are large, combining square jaw surgery with masseter muscle resection can be very helpful.

As shown above, combining square jaw surgery and masseter muscle resection can improve the bulky appearance of the lower jaw.
If the square jaw is not large and only the muscle is the issue, Botox alone can be sufficiently effective.
In this way, a peanut-shaped face can be transformed into a smoother face shape through several combined procedures.
