There is a contouring surgery that takes about 30 minutes and allows for a quick return to daily life after surgery.
That surgery is the 230-degree zygoma reduction surgery.
Today, I’ll correct some misunderstandings about 230-degree zygoma reduction surgery and provide more accurate information so that those currently considering surgery can make the right choice.
230-Degree Zygoma Reduction Surgery
The 230-degree zygoma reduction surgery, performed through a scalp incision, reduces the side cheekbone by the same amount as a general 3D zygoma reduction surgery.
Of course, because there is no resection of the 45-degree cheekbone area, the amount reduced in the 45-degree area is different, but that does not mean there is no effect.

The CT image you see is of the facial bones viewed from below looking upward.
In 230-degree zygoma reduction surgery, the 45-degree cheekbone area is not completely cut; only part of it is osteotomized, while the side cheekbone is fully cut.

If you look at the CT after surgery, you can see that the side cheekbone has moved inward, and the absorbable pins do not appear on the CT as if they were not fixed, but they are firmly secured.
Of course, if the side cheekbone were simply pushed in and the procedure ended there, the 45-degree cheekbone would remain. However, in patients with a prominent 45-degree cheekbone, the partially osteotomized 45-degree area is shaved to reduce its volume.


As you can see, not only has the side cheekbone been moved inward, but the 45-degree cheekbone has also been smoothly shaved, reducing its volume.
I’ll also show you an actual patient’s photos.

This patient underwent 230-degree zygoma reduction surgery.
If you compare the before-and-after photos, you can clearly see that the 45-degree cheekbone has moved inward.
As you can see, 230-degree zygoma reduction surgery is not a procedure that only affects the side cheekbone. It can push the side cheekbone inward and reduce the overall cheekbone volume through shaving of the 45-degree area.
How Is It Different from 3D Zygoma Reduction Surgery?
3D zygoma reduction surgery is more effective for people whose cheekbones are significantly developed because it is a procedure that removes part of the cheekbone by resecting it.





For patients whose 45-degree cheekbone is especially prominent, or for those who want to reduce the cheekbones as much as possible regardless of the anesthesia method or swelling, I would recommend 3D zygoma reduction surgery. However, if a patient can be sufficiently addressed with 230-degree zygoma reduction surgery, I do not think there is a need to undergo 3D zygoma reduction surgery unnecessarily.
