Today, I’d like to look at one of the questions patients ask most often during consultations: “How long does the facial line keep changing?” We’ll go through this based on real cases.
Contour surgery is not simply a procedure that reshapes bone; it is a process in which the face’s structure and soft tissue adapt together over time.
That is why, as time passes, the result often becomes more natural and softer in appearance than it does right after surgery.
Does all the swelling disappear by 6 months?
Textbook standards vs. real-life changes
I previously wrote a column about how swelling changes after contour surgery.
Medically speaking, most swelling is said to have nearly disappeared by 6 months after surgery.
But in real follow-up observations of patients, there are many cases where the facial line becomes a little smoother even after 6 months, and where it is felt to have improved even more after 1 year.
This is not simply because the swelling goes down; it is also because the muscles adapt to the new contour line, and the skin and fat settle into place in a stable way.
So while swelling may be considered over at 6 months, the completion of the facial line can take more than a year.
Then, over the year after surgery, how does the face change?
Because the face changes day by day after surgery, patients who follow the recovery process closely often say, “Time is the best medicine.”
Below is a summary of the changes from day 1 to 1 year after surgery in a real patient.

Before surgery - A face that looks long and large

Day 1 after surgery – The stage of swelling and tightness right after surgery
The day after surgery, everyone is surprised when they look in the mirror.
The face feels puffy and firm, and there is a strong sense of tightness and heaviness around the cheekbones and jaw.
This is the period when swelling reaches its peak, so cold compresses and enough rest are most important.
It is still too early to talk about the facial line.


1 week after surgery – The period when swelling moves downward
About a week later, the swelling on the face moves from the upper area downward.
So the cheekbones may seem smaller, but the area under the chin can look dull and heavy.
From this stage, bruising begins to fade, and daily activities such as eating and washing the face gradually become possible.

1 month after surgery – The contour begins to show
About a month after surgery, people around you often start saying that you look much more natural.
The major swelling has gone down, but deeper tissue swelling may still remain under the cheeks or at the chin tip.
At this stage, you can think of the face as being about halfway through the swelling process.
If you compare photos, you can clearly feel the change in the contour line from the 1-month mark.


3 months after surgery – A time for natural expression and lines
After 3 months, there is almost no burden even when going out or taking photos.
The jawline becomes smoother, muscle tension relaxes, and facial expressions soften, so people around you often ask, “Did you lose weight?”
At this stage, almost no trace of surgery remains.

6 months after surgery – The stage when swelling ends and the shape stabilizes
Textbook-wise, this is considered the end of swelling.
But in real life, this is also where the real change begins.
The skin and subcutaneous tissue settle, and as the cheekbone and jawline connect more naturally, the facial line becomes more refined.
This is also the stage when patients themselves feel, “My face feels lighter now.”

1 year after surgery – The stage of a completed facial line
One year after surgery, most patients say, “Now it really feels like my face.”
The swelling has completely disappeared, and as the tissue fully adapts to the new structure, the most natural and three-dimensional contour is completed.
The jawline that initially looked angular becomes smoothly connected, and the cheekbone line begins to harmonize with the entire face.
Through consistent care and follow-up, the most natural and beautiful facial line is completed between 6 months and 1 year.

It definitely seems that changes are visible even between 6 months and 1 year.

The result of facial contour surgery is not only what you see immediately after surgery.
The entire process in which the tissue heals and becomes harmonious over time is part of the surgery.
So even if there is still swelling after surgery, or if the result feels a little disappointing, please do not rush; it is better to trust the recovery process and watch it over time.
