Hello.
I am Ahn Young-seok, CEO and head dentist of Make Dental Clinic.
Many people worry when the gums around previously treated laminates
become darkened or when a black line appears at the boundary
between the teeth and the restoration.
A procedure that was clearly meant to make the teeth look better
can instead make the face seem darker or create the feeling
that there may be a problem with gum health,
which can be distressing.
Several combined factors can contribute to
this phenomenon.

First, there is the issue of the material.
In the past, materials containing metal components were often used.
Over time, metal ions can seep out and darken the gums,
or shadows from metal prosthetics can show through,
making them look like a black line.
Second, there is the issue of the technique or design
used at the time of treatment.
If the laminate does not fit tightly against the gums and a
microscopic gap forms, plaque or food debris can accumulate there
and appear dark.
Third, there is a change in gum health.
As the gums recede, the tooth reduction surface under the laminate
may be exposed, or the edge of the prosthetic may become visible,
making the area appear dark.

- What changes happen to old prosthetics and the gums?
Teeth and gums are tissues that continue to change over time.
On the other hand, prosthetics are made from artificial materials,
so differences may appear as time passes and natural changes occur.
In particular, the gums tend to gradually recede with age.
During this process, the boundary area of a tooth that was once
covered by laminate, or the reduced tooth surface, may become exposed.
If a prosthetic containing metal was used in that area, or if the tooth itself
is dark in color, the boundary area may look darker.
We carry out customized tooth design that considers the patient’s facial shape,
lip line, and smile habits.
- Is it simply an esthetic issue?
Discoloration or the black line seen around laminates
is often thought of only as an unattractive esthetic issue.
However, in some cases, it also needs to be considered as a sign that
may indicate changes related to oral health.
If the discoloration is related to an old metal prosthetic,
checking the condition of the prosthetic may be helpful.
Also, if a microscopic gap has formed at the boundary with the laminate
and plaque has accumulated, bacteria may enter through that area,
potentially leading to cavities or inflammation.
If this condition continues, it can place a burden on gum health,
so caution is needed.
Make Dental Clinic was founded with the hope that no more patients
would have to suffer from unnecessary tooth reduction.
- The start of retreatment is an accurate diagnosis
If discoloration or a black line appears due to an old laminate,
the starting point for retreatment is an accurate diagnosis.
The medical team examines the overall condition of the mouth and checks
the existing material, design, and bonding status.
Through X-ray imaging, they assess the condition of the tooth under the prosthetic,
changes in the jawbone around the gums, and whether the prosthetic fits properly.
They also evaluate whether there is inflammation or gum recession through an oral examination.
In addition, through sufficient consultation about past treatment history and the current discomfort,
they determine a retreatment plan suited to the individual’s condition.

- Careful removal is necessary.
To proceed with retreatment, the existing laminate must first be removed.
The important point here is that removal must be performed while fully considering the current condition of the teeth.
Because it is firmly bonded to the tooth surface, excessive force during removal can place stress on the natural tooth underneath.
In particular, if the bonding was especially strong in the past or if a relatively large amount of tooth structure was removed, an even more careful approach is needed.
Therefore, the medical team uses various tools and methods to separate it step by step in order to reduce stimulation to the teeth,
and carries out removal in a way that minimizes tooth damage.
Through 충분한 communication with each patient,
we achieve the most satisfying results.
- How is it designed to be gum-friendly?
During retreatment, to prevent the same problems from recurring,
it is important to prioritize gum health in the design.
First, the principle of minimal reduction.
Unnecessary tooth reduction can burden the teeth and gums,
so treatment is planned within a range that preserves the natural tooth.
Second, precise margin setting.
The edge of the laminate should be adjusted so that it fits tightly to the tooth
without extending excessively under the gums.
A smoothly formed margin at an appropriate position above the gum line helps reduce irritation
and prevent plaque buildup.
Third, the importance of material selection.
Using all-ceramic materials without metal components can reduce the possibility of discoloration,
and a result that is more harmonious with the natural tooth can be expected,
with similar color and translucency.
Make Dental Clinic considers not only the result, but also satisfaction with the process.
As we have seen today,
discoloration or a black line caused by old laminates may not be merely an esthetic issue.
These changes may be the result of several combined factors,
such as aging prosthetics, recession, and microscopic gaps at the bonding area,
and they suggest that the current condition of the mouth should be checked.
Therefore, if you see discoloration or a black line around a restoration that was placed in the past,
it may be helpful to visit a dental clinic and get a consultation rather than worrying alone.
If treatment is carefully planned based on sufficient information and consultation with the medical team,
and followed by consistent care afterward, you may be able to expect a more natural and healthy smile.
Thank you.
