Hello.
I’m Youngjin Wi from Beauty’s Doctor.
Recently,
as more and more people have wanted
to create a Western-style, firm buttock line,
many people have encountered
frightening cases involving the BBL,
which is famous overseas,
and have begun searching for buttock Sculptra
as a safer alternative.
On internet communities and social media,
you often see statements that Sculptra is
“a safe treatment that can perfectly replace
the dangerous BBL.”
To give you my very honest conclusion first,
Sculptra is indeed a treatment whose mechanism
within the body is completely different from that of a BBL.
However, due to the anatomical characteristics
of the buttocks,
there are inevitably separate,
more challenging variables than with facial treatments.
For those who are researching buttock-volume treatments
for the first time,
I’ll briefly and clearly organize the medical facts
that are easy to find confusing before comparing prices.
BBL and Sculptra Work in Completely Different Ways

First, a BBL (Brazilian Butt Lift)
is a surgical procedure in which a large amount of fat
is liposuctioned from your abdomen or thighs,
then reinjected in large quantities
into the buttock layers to create the desired shape.
During this process, accidentally introduced fat can enter
the blood vessels and block the lungs or brain,
causing fatal systemic accidents such as
“fat embolism” or “inferior vena cava injury.”
The risk of these complications is among the highest
of all cosmetic plastic surgery procedures,
which is a serious disadvantage.
By contrast, buttock Sculptra does not work by
forcibly filling volume with large amounts of
artificial synthetic filler or another person’s fat.
It is an injectable treatment that involves injecting
PLLA (Poly-L-Lactic Acid) to induce the skin
to produce its own collagen,
promoting gradual volume formation over time
as collagen production occurs within the body.
Because it does not involve surgical incisions
or large-volume fat grafting,
it has the academic characteristic of being able
to completely exclude the risk of fatal complications
such as systemic embolism associated with surgical methods.
Even So, the Buttocks Are a Medically Challenging Area

Regardless of the safety of Sculptra’s ingredients themselves,
the buttocks have anatomical characteristics that make them
very large in area and frequently exposed
to strong pressure in everyday life.
Because strong, sustained pressure from body weight
is applied whenever we sit on a chair or lie down,
there is a much higher possibility than with typical facial treatments
that the injected PLLA powder will clump on one side
or form nodules that feel like hard granules.
For this reason, aftercare to evenly spread the injected material
under the skin can be a hundred times more important
than it is for the face.
For five days after the treatment, massage the treated area carefully
five times a day, for five minutes each time.
Strictly following this “5-5-5 massage rule” at home
is essential for helping distribute the injected particles evenly
and reducing the possibility of nodules.
In addition, during the first few days after treatment,
avoid sitting on hard chairs for long periods whenever possible,
and make an effort to minimize direct physical pressure
on the skin as much as possible.
Plan the Recommended Number of Treatments and Your Expectations in Advance

Buttock Sculptra is not completed with a single treatment.
In general, a treatment plan involves gradually accumulating
at least three to five or more sessions,
usually spaced four to six weeks apart.
Rather than expecting your buttocks to become
immediately fuller the day after an injection,
you should view this as a long-term treatment
in which soft, curved contours gradually emerge
over a period of at least six months to one year.
The phrase used in some marketing advertisements—
“dramatic hip-lifting results like BBL surgery
in just one treatment”—
is an exaggerated statement that fundamentally misunderstands
Sculptra’s cellular regeneration mechanism from a medical perspective.
If the congenital indentation and sagging of the hips
are particularly severe,
or if overall volume is significantly lacking,
there are clear medical limitations to filling the area
with Sculptra injections alone.
Therefore, from the initial consultation,
you should honestly discuss with your doctor
“whether the current degree of sagging
falls within the range that can be corrected
with injectable treatment,
or whether your body type is better suited
to autologous fat grafting surgery.”
This makes it easier to manage expectations
and avoid disappointment.
This article summarizes general information
to help you understand body-contouring treatments.
The most accurate and safe treatment suited
to your actual buttock muscle structure and skin laxity
should be decided through an in-depth consultation
with a doctor who has directly assessed your body type.
Frequently Asked Questions (FAQ)

Q. If I get Sculptra just once,
will the indentation along my hip dip become noticeably fuller?
A. For a few days immediately after treatment,
you may feel some increased volume due to the fluid
injected along with the treatment.
However, this fluid is quickly absorbed by the body
and disappears.
The true changes from Sculptra, as mentioned earlier,
begin to appear gradually over three to six months
after receiving at least three accumulated injections,
as collagen is produced within the skin.
Therefore, this treatment requires patience
and a long-term perspective.
Q. When can I begin buttock exercises such as squats
after receiving the treatment?
A. Light walks around your neighborhood
or ordinary walking are generally possible
starting the day immediately after treatment.
However, you should avoid heavy weight training
at the gym or intense aerobic exercise
for at least three to five days after treatment.
In particular, to prevent nodules caused by granules
clumping beneath the skin,
you must be especially careful during the first week
after treatment with exercises that cause significant
physical impact or friction on the buttocks.
Q. Is it a legal treatment to receive a large-volume procedure
at a dermatology or plastic surgery clinic in Korea?
A. The Sculptra (PLLA) product itself is a legal medical preparation
that has officially received approval from
the Ministry of Food and Drug Safety (MFDS) of the Republic of Korea
and the U.S. FDA.
However, the treatment area and the amount to be administered
are determined according to the treating doctor’s
clinical judgment and expertise.
Because the injection volume is larger for the buttocks than for the face
and the anatomical vascular structure must be understood precisely,
you should be sure to choose a nearby medical institution
with extensive clinical experience in large-volume body treatments.
This is how to help prevent unexpected side effects
and ensure safety.
[Post-Treatment Precautions and Side-Effect Information]
After body Sculptra (PLLA) injections,
temporary swelling, pain, bruising,
redness, and tenderness may occur
depending on the individual’s constitution.
Because the buttocks are continuously exposed
to physical pressure,
there is a relatively higher risk than with facial treatments
of delayed nodules and granulomas,
in which the injected material clumps
and hard granules can be felt.
Infection or an inflammatory reaction
may also occur through the injection needle.
If unusual symptoms persist after treatment
or you experience severe pain,
you must immediately visit the medical institution
where you received the treatment
and receive medical attention.
