Do you know the number one cause of tooth loss in Korea?

It is periodontitis.

Periodontitis has little to no pain until the symptoms become severe,

which is why it is sometimes called a silent disease.

Because there is no major pain or discomfort, many people miss the right time for treatment,

and it often ends up leading to full extraction.

Because full-mouth implants require placing a large number of implants,

the long treatment period and the burden of frequent dental visits

can become a concern.

For those who have searched extensively online,

if implants are placed after extraction,

many believe that implant surgery can always be completed within 3 months,

so I prepared today’s post.

If immediate placement is performed after extraction,

can treatment really be finished within 3 months?

Let’s take a closer look.

In the intraoral photos, you can see pus coming out from multiple teeth.

This is a common symptom seen in patients with chronic periodontitis.

In addition, healthy gum color is a bright pink,

but the areas where pus is coming out show that, due to severe periodontitis, the gum color has turned dark red.

Looking at the X-ray images,

you can see that the jawbone has generally resorbed significantly,

and that bone loss has progressed greatly in the right maxillary molar area, where the teeth are missing.

Implants are a treatment method that achieves strong chewing force by fusing with the jawbone,

so they are greatly affected by the condition of the jawbone.

For immediate placement after extraction to be possible,

the condition of the extraction socket must be favorable.

The extraction socket refers to the space where the tooth used to be.

If an implant is placed while inflammation remains in this socket,

the inflammation can cling around the implant and lead to surgical failure.

Therefore, when the inflammation inside the extraction socket is deep and difficult to reach with curettage instruments,

immediate placement is not possible,

and a healing period for the jawbone, during which we wait for the inflammation to subside naturally, is needed (2–3 months).

Fortunately, the patient was not in a state of such deep inflammation that immediate implant placement after extraction was impossible,

so we decided to proceed with full-mouth implant surgery by dividing it into the upper and lower jaws.

Even if the inflammation was severe before surgery,

if the inflammatory tissue in the extraction socket can be thoroughly scraped away,

immediate placement after extraction is possible. ^^

Because immediate placement means placing the implant right after extraction,

doesn’t that mean treatment is almost finished? Many people think that once an implant is placed, all treatment is over,

but implant surgery is divided into first and second surgeries,

for a total of two operations.

The first surgery is the one we know, where the implant is placed into the jawbone,

and the second surgery is the one that exposes the implant through the gum surface.

If you look at the patient’s X-ray image,

you can see that some implants are inside the jawbone,

while others are exposed outside the gum;

this difference is exactly the difference between first and second surgery.

Implants have a better prognosis when they undergo osseointegration while receiving as little stimulation as possible.

That is why, in areas where a large amount of bone graft material was used,

a new bone is formed,

and after placing the implant within the jawbone so that the bone and implant can fuse well,

the area is sutured. (= first surgery)

When the first and second surgeries are performed separately,

the second surgery is carried out after a recovery period of 2–3 months.

Just because it is immediate placement

does not mean that all treatment steps can always be done at once on the same day.

(Usually, there is a recovery period of 3–4 months, but because the patient had a large amount of bone graft material placed,

a further 2-month recovery period was added.) Cases in which all treatment steps can be completed on the same day are

cases where some of the initial jawbone remains,

or the bone quality is favorable enough to secure the implant’s initial stability.

Implants are artificial teeth that, once placed,

are used for nearly 10 years or more.

For implants that are used for such a long time, is rushing treatment always the right answer?

I do not think that rushing treatment is always the best approach.

The best approach is to remove as many risk factors as possible

and provide treatment in a way that allows for a good outcome, right?

So when deciding on implant treatment,
