Have you heard the term “ongni”?
Okni and ongni have different meanings.
Okni usually refers to crooked, protruding teeth,
and ongni refers to teeth that are positioned inward.
Because the dental arch is drawn inward,
it does not look aligned when you smile.

Also, teeth support the lips.
Since ongni means the teeth sit back, they cannot properly support the lips,
which creates the impression that the lips are curling inward when you smile or speak.
That is why patients with ongni often say,
“My smile looks strange because of my teeth.”
Today, we prepared a case in which this inward-leaning tooth position was improved with laminates.

This is the initial intraoral photo.
The patient had front teeth that leaned inward, giving the lips a flat appearance.


Inward-leaning teeth are often improved with orthodontic treatment or laminates.
Since the patient did not want orthodontic treatment, which takes a long time,
we planned treatment to improve the tooth color more quickly with laminates as well.
Which cases of inward-leaning teeth can be improved with laminates?
Among inward-leaning teeth, cases where the teeth bite in the opposite direction are better treated with orthodontics.
It is better in the long term to correct the overall bite,
relieve temporomandibular joint pain or overload,
and establish a normal bite.

However, in cases like this patient’s, where there is no bite-related problem,
the main goal is aesthetic improvement rather than function.
So, laminates can be a good choice because they can align the teeth neatly
while also improving tooth size and color at the same time.


First, to improve inward-leaning teeth,
we need to make the shape and thickness of the prosthesis slightly
different from a standard laminate.
The prosthesis near the gumline, in the cervical area, is made thinner,
and it is made slightly thicker toward the tip of the tooth, the incisal edge,
so that natural gum support is created and the lips are also supported.
I want my teeth to look brighter and larger.
This is what the patient said during the treatment planning consultation.
To make the teeth appear longer and larger as requested by the patient, the healthy gum tissue above the teeth must be
slightly contoured with a laser to adjust the tooth length.
This method is commonly called “gum contouring.”

It is often performed during laminate treatment to match tooth height,
or when patients with short teeth, like this patient,
need their tooth length improved for a brighter, more refreshed look.

Since patients with well-aligned teeth only need tooth reduction equal to the thickness of the prosthesis (0.3 mm),
you can receive treatment with preservation of the natural teeth.
Because the gums are gently contoured with a laser,
it cuts with heat while also stopping bleeding at the same time.
So there is less bleeding, less tissue damage, and recovery happens very quickly. ^^

Even though the photo was taken the same day, you can see that the contoured gum area is not severely swollen
and there is no accompanying bleeding.
The gum soft tissue is also one of the fastest-healing parts of the body.
This photo was taken when the patient returned five days later for mandibular laminate treatment,
and you can see that the gum contouring area had healed completely. ^^
The patient said one month after treatment
that they were so happy with the treated teeth
that they wanted to proceed with the lower teeth as well, where there was a slight difference in tooth color. ^^

The teeth that had looked as if they were curling inward
now maintain normal volume
and can be seen to look very natural.
For the lower teeth, the patient had already called in advance to request same-day treatment,
so on the day of the visit, tooth prep, laminate fabrication, and bonding were all completed.
