Today, we would like to share the treatment case of a patient who underwent both challenging, high-level implant revision surgery and full-mouth implant treatment at the same time.

Let us first look at the patient’s initial intraoral photographs.

The patient was using a complete denture in the upper jaw and a partial denture in the lower jaw.

The implants that had been supporting the partial denture in the lower jaw began to loosen,

and the patient said that eating had become difficult in their current condition.

The patient was in their 60s and had traveled from outside the Seoul area to visit our clinic on the recommendation of an acquaintance.

Let us begin with the upper jaw.

Because the patient had been without teeth in the upper jaw for a long time, the maxillary sinus had enlarged downward due to maxillary sinus pneumatization.

Bone resorption was also present, leaving insufficient alveolar bone to place implants with adequate stability.

The upper molar area is subject to strong chewing forces, so implants with a typical length of 7–10 mm are generally placed in this area.

Because the available alveolar bone was not long enough to place implants of this size,

we decided to perform bone grafting along with a sinus lift, followed by implant placement.

As for the lower jaw,

inflammation had developed around all four implants that had been used as abutments for the lower partial denture.

As a result, the implants were severely loose, and bone loss had also occurred around them.

Because the patient wanted full-mouth implant treatment, we established a treatment plan to remove the inflammation and then replace the implants.

The implant surgeries were scheduled about one week apart, with treatment performed in the order of upper jaw → lower jaw.

Can’t all the implants be placed in one day? Why is the treatment divided into multiple sessions?

Performing bone grafting and implant placement in all areas in a single day would place a considerable physical burden on the patient. In addition, even if the implant placement were performed as quickly as possible, the anesthetic would not last for the entire duration of the surgery.

At the same time, it would not be possible to administer an excessive amount of anesthetic medication at the risk of placing undue strain on the body.

Above all, the patient’s safety was the most important consideration, so the director explained this point in detail in person before treatment began.

Fortunately, apart from the areas undergoing implant revision surgery, the patient did not have severe inflammation in the alveolar bone.

Because stable initial implant fixation was achieved,

both stages of surgery were completed on the same day to reduce the number of visits and shorten the treatment process.

After undergoing implant revision surgery and full-mouth implant treatment in this way,

how long did the entire treatment period take?

All treatment was completed over a total period of five months.

Although performing the first and second surgeries at the same time helped shorten the treatment period,

it was also possible to manufacture the prostheses quickly through consultation with the director at our in-house dental laboratory.
