Hearing that spots seem to be increasing may sound like a purely cosmetic concern, but from my perspective, it is something that needs to be examined a little more deeply.

That is because the treatment method can be completely different depending on what the condition called a “spot” actually is.

So today, I’d like to explain directly what types of spots there are and how treatment methods differ for each symptom.

Types of spots

“Spots” is a broad term for lesions that appear as darker pigmentation on the skin surface, but it includes various conditions with different structures and causes.

Representative examples include melasma, freckles, lentigines, age spots, and post-inflammatory hyperpigmentation.

Melasma

� Melasma is mainly caused by hormonal changes, excessive sun exposure, and genetic factors, and the main cause is an increase in melanin pigment in the dermis and epidermis.

It usually appears symmetrically on the forehead, cheeks, and upper lip, and is characterized by occurring without relation to inflammation.

Freckles

Freckles are generally observed from childhood and tend to be especially sensitive to genetic influence and sun exposure.

They are caused by excessive melanin production limited to the epidermal layer of the skin, and they usually appear on the cheeks and around the nose.

Lentigines

Lentigines are brown spots that develop after adulthood, and can be seen as the result of sun exposure and skin aging.

They are larger and darker than freckles and are related to the proliferation of melanocytes within the epidermis.

Post-inflammatory hyperpigmentation

Post-inflammatory hyperpigmentation refers to brown pigmentation left behind by inflammation after acne, wounds, or laser procedures.

Depending on skin type, it can persist for a long time, and the main cause is an overreaction of melanocytes.

Age spots

Age spots are benign epidermal tumors that appear with age, and they are often mistaken for spots because they can look brown or black.

They can be removed with a laser, but they need to be approached a little differently from other pigment lesions.

This is one of the questions many people ask.

The most important first step in spot treatment is an accurate diagnosis.

If it is difficult to distinguish by the naked eye, a skin diagnostic device should be used to check the depth and extent of the lesion.

Melasma

For example, melasma often has pigment spread into the dermis, so it can react sensitively to strong lasers, and if laser treatment is performed too aggressively, the condition may actually worsen.

On the other hand, lentigines and freckles are usually limited to the epidermal layer, so in many cases they can be treated with relatively strong energy without issue.

In addition, a person’s skin type, existing skin condition, presence of inflammation, and history of previous treatment must also be considered, so I want to emphasize that self-diagnosing based only on the color or shape of the lesion is very dangerous.

The procedure commonly used these days for spot removal is laser treatment.

However, not all lasers work the same way.

Depending on the depth and type of the pigment lesion, the wavelength, pulse width, and output intensity of the laser must be adjusted differently.

Pico laser

For example, pico lasers use extremely short pulses measured in picoseconds to finely break down pigment.

Because they can target pigment in both the epidermis and dermis at the same time, they can be effectively used for mixed spots such as melasma, freckles, and post-inflammatory hyperpigmentation.

Conventional nano lasers, which are not pico lasers, are mainly used for freckles and lentigines.

Devices such as IPL are also widely used.

Strictly speaking, it is difficult to call it a laser, but it has the characteristic of helping improve epidermal pigment through photodynamic action and contributing to brightening the overall skin tone.

It is suitable for superficial epidermal pigment lesions and can be applied evenly over a wide area.

Because each laser has different characteristics, you need to receive a clear diagnosis of your own skin condition and pigment lesion before choosing the most suitable one.

If necessary, chemical peels, oral medication, or topical medication may also be used together.

� The single word “spots” includes various lesions, and their causes and treatment methods are also very different.

Rather than expecting all spots to disappear after just one laser session, it is most important to establish an accurate diagnosis and a customized treatment plan.

Also, to help prevent recurrence, sun protection is essential, and you should remember that ultraviolet rays are present even indoors.

We recommend developing the habit of reapplying sunscreen frequently according to the situation, rather than only once or twice a day.

Spot treatment is not a short sprint; it is a long-distance marathon.
