At Songpa Station Dental Clinic,
with years of clinical experience,
we sometimes come across unusual wisdom tooth cases.

Sometimes they are even discovered
in unexpected places like this...
In this case, the patient had pain every time
they chewed food and felt like their tooth was loose,
so an examination was performed.
Through a detailed examination, around the upper left
molar on the patient’s left side,
severe inflammation had developed around the tooth root,
and the alveolar bone (the bone that supports the tooth)
had been resorbed.
This condition was the main reason
the tooth felt loose.
In addition,
around the apical (root tip) area of that tooth,
there was an abnormally positioned
impacted wisdom tooth.
When analyzed at Songpa Station Dental Clinic,
as the lesion in the molar grew larger,
it is believed to have caused changes in the surrounding structures,
resulting in a shift in the developmental position.
This can happen sometimes.
As an intraoral lesion grows larger,
it can exert pressure on surrounding tissues and structures,
or destroy the surrounding bone tissue,
which can change the development direction and position
of an impacted wisdom tooth.
The diagnosis showed that the severely inflamed posterior molar
had to be extracted,
and the plan was to restore the area with an implant.
However, the abnormally positioned wisdom tooth
was interfering with the implant placement path,
so that area also needed to be removed.

At Songpa Station Dental Clinic,
while analyzing the exact location through CT imaging,
it was found that part of the wisdom tooth
was in contact with a structure called the maxillary sinus.
The maxillary sinus is an air-filled cavity located
between the cheekbone and the nose,
connected to the nasal cavity,
and if this structure
is perforated, various complications can occur.
If this wisdom tooth, which was in contact with that structure,
were removed...
there was a risk of perforation.
However, even if a perforation occurs,
through appropriate closure procedures,
if it is properly managed,
a stable result can still be achieved.
Therefore, after explaining this thoroughly,
treatment was carried out.
During the extraction process, as expected at Songpa Station Dental Clinic,
quite a large amount of inflamed tissue came out.
In cases like this, it is important to remove the inflamed tissue completely.
If any inflamed tissue remains,
it can interfere with the healing process.
Therefore, all inflammation was carefully curetted out
and the area was thoroughly disinfected
to create a clean environment.
As expected, part of the maxillary sinus was perforated
during the wisdom tooth extraction,
but closure was performed immediately
to prevent further complications.

In addition, to restore the bone space
that had been destroyed and lost,
socket preservation was performed together.
This is a process in which the space created after extraction
(the extraction socket) is filled with bone graft material,
to minimize further resorption
and promote healing.
It is important for rebuilding the lost bone
and maintaining its quantity and quality
to prevent further bone resorption.
Because a large amount of bone had been lost
due to inflammation,
we waited with enough time for proper healing.

1 month later
At regular intervals, follow-up examinations were conducted
to check the healing status
and determine the timing for implant placement.

After sufficient healing for about 4 months,
the implant fixture was placed.

Because there is a limit to restoring the alveolar bone height
that had previously been lost in large amounts,
Songpa Station Dental Clinic had no choice but to place it somewhat deeply
while finding a stable area.
However, this is not a problem.

In cases like this, a customized implant prosthesis
suited to the patient’s oral condition
can be made and completed stably.

In this way, the complicated case caused by
severe inflammation and the abnormally positioned wisdom tooth
was successfully completed.
It is currently being maintained well
without any problems.
As you can see from what was introduced today at Songpa Station Dental Clinic,
when signs are found that the bone surrounding a tooth is being destroyed,
it is important to proceed with treatment without delay.
The longer the problem is left untreated,
the more complicated and time-consuming the treatment becomes,
and healing may be delayed,
causing even more hardship.
Also, in cases of deeply impacted wisdom teeth like this,
if they are in contact with important structures such as the maxillary sinus,
it is safer to receive treatment from an experienced specialist
with sufficient experience and skill.
Above all, rather than getting an examination only after pain develops,
it is important to detect problems early
through regular checkups
and receive treatment at the appropriate time!!
