When treating patients who visit the dental clinic because of wisdom tooth problems, we sometimes encounter unexpected cases.

Most wisdom teeth are impacted or positioned in a way that pushes against adjacent teeth,

but in rare cases, the tooth itself may have developed in a form that differs from normal.

Today’s case is one such example.

This is the story of a female patient in her 20s who visited us because of pain in her lower right wisdom tooth.

Wisdom teeth are often buried beneath the gums, and it is difficult to determine the tooth’s complete structure based only on what is visible from the outside.

In particular, when the roots of a wisdom tooth are close to the nerve canal, extra care is needed when planning the extraction.

In this patient’s case, the root tips were close to the nerve canal, so we first performed a CT scan before the extraction.

After analyzing the CT scan, we found that the wisdom tooth did not have a typical structure; instead, it consisted of two teeth joined together.

This type of tooth is called a fused tooth or a twinning tooth.

Although such cases are reported relatively more often in the lower front teeth,

they are extremely rare when found in the wisdom tooth area.

If we had proceeded with the extraction in this case without taking a CT scan,

the unexpected structure could have made the extraction more difficult.

However, because we first checked

what the tooth’s shape was,

how close it was to the nerve canal,

and which direction would be best for removal,

we were able to establish an extraction plan using a safe approach.

When a wisdom tooth is large or broadly impacted, it is often necessary to remove a significant amount of gum bone during extraction or divide the tooth into several pieces before removing it.

However, in this case, after sufficiently analyzing the CT scan,

we considered the tooth’s shape and direction and proceeded with the extraction without dividing the tooth, as far as possible.

As a result, the procedure was less burdensome for the patient, and recovery was also faster.

Looking at the extracted tooth, you can see that two wisdom teeth were actually joined together.

The patient was also surprised to see the tooth after the extraction, and since this is not a case I encounter often, I remember treating it with particular interest.

After the extraction, we took an X-ray to check the extraction site and surrounding structures again.

Although the tooth had been located close to the nerve canal, the extraction was completed within the planned range,

and treatment was completed stably without any unusual findings.

As this case shows, the shape and number of wisdom teeth, as well as the difficulty of extraction, vary from person to person.

In particular, when a tooth is close to the nerve canal, deeply impacted, or abnormally shaped,

or when it has a rare structure such as a fused tooth, a thorough examination is necessary before extraction.
