Songpa-dong Dentist: It Wasn’t Just the Front Crown That Was Replaced! A retreatment case that also considered the gum line

When a front tooth crown breaks or an old restoration becomes uncomfortable,

many people think,

“Wouldn’t it be enough to have a new one placed?”

Of course, making a new crown is important.

However, there is something that needs to be considered just as carefully as the restoration.

That is the gums.

No matter how good the crown is, if the gum line looks unnatural, it can be difficult to create a natural appearance.

Therefore, today I would like to introduce a case in which a broken front tooth crown was replaced along with gum contouring.

A patient in their 40s who visited us because of a broken front tooth crown

The patient had previously received front tooth crown treatment at another dental clinic.

Part of the inside of the crown had fractured, so they visited us for retreatment.

When examining the initial condition, there was an issue with the crown itself, but another aspect caught my attention first.

A closer look at the patient’s front teeth showed that the gum lines on the left and right were not completely symmetrical.

In particular, the gum on the side requiring retreatment had receded slightly lower than the gum on the opposite side.

As a result, the teeth also appeared to be different lengths.

For aesthetic treatment of the front teeth, matching the tooth color alone is not enough to make the result look natural.

One of the most important factors is symmetry.

As in this case, if the gum heights are different, the result may feel unnatural even if the crowns are made in the same color.

Therefore, I planned to correct the gum line while making a new crown.

Gum contouring was performed after removing the existing crown

First, I removed the existing crown and then performed gum contouring using an electrocautery device.

A temporary tooth was then fitted.

Even after gum contouring, the final crown is not made immediately.

This is because the gums gradually stabilize over time.

For this reason, a temporary tooth was made for the patient to use for approximately two weeks.

During this period, the gums can naturally settle, while the space and shape needed for the final crown can be established more stably.

We were also able to confirm that the gum lines, which were the goal of this treatment, had become fully symmetrical.

The final crown was made after the gums stabilized

Once the gums had stabilized sufficiently, the final crown was made.

The final restoration was designed to harmonize as closely as possible with the opposite front tooth.

The tooth length, tooth shape, gum height, and color were all taken into consideration.

Compared with the initial condition, the final result showed harmony not only in the tooth shape and proportions but also in the gum line.

This allowed us to complete front teeth that looked much more natural than before.

The patient had thought that achieving left-right symmetry would be limited because of the existing crown.

However, they were very satisfied after seeing how naturally the gum line and tooth height had been balanced after treatment.

This was also a particularly rewarding case for me.

The front teeth are the area people notice first when smiling or speaking.

Therefore, when a crown breaks, it is important not to simply replace the restoration.

The gum height and shape, harmony with the surrounding teeth, and the overall balance visible when smiling should also be considered together.
