The front teeth are the first area people notice when meeting someone face-to-face.
That is why even slight discoloration or visible spaces between the teeth can cause more stress than expected.
In consultations, I often hear people say:
“I do not want to smile because my teeth look yellow.”
“The space between my front teeth makes me look older.”
More people say these things than you might expect.
Tooth discoloration and black triangles are each concerning on their own, but when they occur together, aesthetic concerns can become even greater.
Today, I would like to introduce a veneer treatment case involving a patient who visited us with these concerns.
Front teeth with severe discoloration and black triangles

This patient said that the discoloration of their front teeth had been severe for a long time and that they felt self-conscious whenever taking pictures or smiling.
Intraoral photographs showed a shade different from the surrounding teeth, particularly around the incisal edges.
Black triangles, which appeared as dark spaces between the front teeth, were also visible.

These features were even more noticeable when the patient smiled from the front and side.
The discoloration made the color of the front teeth appear uneven, and the black triangles also created an aesthetic concern.
I focused on “harmony” rather than “color”
When many people develop tooth discoloration, the first treatment they think of is whitening.
Depending on the cause and severity of the discoloration, whitening alone may provide some improvement in certain cases.
However, this case was somewhat different.
The discolored area was extensive, the boundary between the shades was distinct, and black triangles were also present between the front teeth.
Even if whitening made the teeth brighter, the spaces between the teeth and the shape of the front teeth would remain unchanged.
As a result, the overall appearance visible when smiling could still feel awkward.
Therefore, I determined that it was important to improve not only the color of the teeth but also their shape, proportions, and the spaces between the front teeth.
After discussing the options thoroughly with the patient, we planned veneer treatment.

Veneers are an aesthetic treatment in which thin ceramic material is attached to the surface of the teeth.
They can take into consideration improvements in tooth color, refinement of tooth shape, reduction of black triangles, and balance and harmony between the left and right sides.
However, making the teeth unnecessarily large in order to eliminate black triangles can make them look unnatural.
For this reason, before treatment, it is important to plan the size and shape of the veneers while considering the patient’s facial shape, smile, and balance with the surrounding teeth.
For this patient as well, we designed the veneers so that the teeth would not appear excessively large while the spaces between the front teeth could be improved naturally.
How did things change after treatment?


In the photographs after treatment, the discoloration of the front teeth had improved naturally, and the black triangles between the teeth had also been reduced.

Most notably, there was a significant difference in the patient’s smile before and after the front-tooth veneer treatment.
Before treatment, attention was drawn to the discoloration of the front teeth and the spaces between them.
After treatment, the front teeth blended together naturally, creating a much brighter and more comfortable-looking smile.
The patient was also pleased, saying, “Now I no longer feel conscious of my front teeth when I smile.”

What I wanted to show through this case is that veneers are not a treatment intended only to brighten the color of the teeth.
If the shape and proportions of the patient’s teeth, as well as the spaces between the front teeth, are considered together during the design process, veneers may help create a more natural appearance.
Of course, not every case of discoloration or black triangles is suitable for veneer treatment.
It is important to thoroughly review the current condition of the teeth before deciding on a treatment method, including whether tooth reduction can be minimized, whether whitening or composite resin could provide improvement, and whether veneers are more suitable.

As in the case introduced today, when tooth discoloration and black triangles occur together, the color, shape, and proportions of the teeth, as well as the spaces between the front teeth, should all be considered together to achieve a more natural result.
