When addressing tooth discoloration, I consider the following three points important.

Today, I will explain the reasons—or causes—of yellow teeth, how to distinguish them, and how treatment methods may differ depending on the cause.

Yellow Teeth: The Cause Must Be Assessed First

During consultations, many people say that their “teeth are yellow,” but the actual causes are often different.

In some cases, pigments have accumulated on the tooth surface, while in others, the color has changed inside the tooth.

When the tooth surface becomes stained due to strongly pigmented foods or lifestyle habits, such as coffee, tea, curry, wine, or smoking, this is called extrinsic staining.

In contrast, discoloration that develops inside the tooth due to aging, dental trauma, certain medications, or after root canal treatment can be considered intrinsic discoloration.

If the cause is extrinsic staining, improvement may be possible with teeth whitening. However, when the discoloration is intrinsic, conventional whitening alone may not brighten the teeth as much as desired.

Even when the teeth appear to have the same yellowish color, different causes may be involved. Therefore, I first ask when the color began to change and whether all of the teeth changed together.

Understanding the cause first makes it possible to determine the treatment approach that suits your current tooth condition.

Whether all of the teeth are evenly yellow or only one or two are particularly dark can also provide some clues about the cause.

When all of the teeth are evenly yellow, it is often because the enamel—the hard layer covering the tooth surface—has become thinner with age, allowing the color of the dentin underneath to show through.

On the other hand, if only one front tooth is noticeably darker or has a grayish tone, it is necessary to check whether that tooth has previously experienced trauma or undergone root canal treatment.

In fact, during consultations, some patients have said, “I thought all of my teeth were yellow,” but a closer examination revealed that only one front tooth was particularly dark.

In such cases, I believe it is more important to accurately assess the condition of that tooth rather than immediately proceeding with full-mouth whitening.

Checking the cause first, instead of simply matching the visible color, can help reduce unnecessary treatment and select an approach suited to the current condition of the teeth.

Care and Treatment Methods Also Vary Depending on the Cause

If extrinsic staining is the main cause, professional whitening performed at a dental clinic or take-home whitening using custom trays may be considered.

However, there is something that should be checked before deciding which whitening method to choose.

The condition of the enamel, whether there are any cavities, and whether the gums are healthy should all be examined together.

Because tooth sensitivity may occur during the whitening process depending on the condition of the teeth, a pre-treatment examination is an important step that should not be overlooked.

If intrinsic discoloration developed after root canal treatment, a different approach from conventional surface whitening may be necessary.

In such cases, the condition of the affected tooth is checked first, and the treatment approach is then determined together.

In addition, existing restorations such as composite resin fillings or crowns do not whiten like natural teeth, so a color difference may develop between them and the surrounding teeth after treatment.

When discussing whitening, I believe it is more important to first check the current condition of the teeth than to focus on how white they can become.
