As you prepare for front-tooth extraction orthodontics, what criteria are you using to choose a dental clinic in Seocho-dong?

Extraction orthodontics is not simply the process of removing teeth. It is part of an overall plan to move the remaining teeth into the most stable positions.

For satisfactory results, it is important to check whether the clinic follows these three principles:

• Does it first determine whether extraction is truly necessary and whether treatment without extraction is possible?

• Does it explain the decision-making process thoroughly and avoid recommending extraction without sufficient grounds?

• Does it honestly explain the possible side effects during extraction and afterward?

What is the process for front-tooth extraction orthodontics?

The first step is to determine how much space is lacking for the teeth to move.

After examining the length of the tooth roots, the condition of the jawbone, and the condition of the gums through X-rays and an oral scan, we determine whether extraction is truly necessary or whether the issue can be resolved by expanding the dental arch—the space in which the teeth are arranged.

Once extraction is decided on, some of the premolars are usually removed to create space, and the front teeth are gradually moved into that space.

Just as you need to measure the furniture layout in a new home in advance or the door may not close once everything is moved in, extraction without a prior space plan is likely to lead to results that require further correction.

Is extraction always necessary?

Extraction is not necessary in every case.

Extraction is one option considered when there is insufficient space for the teeth to move. If enough space can be created by expanding the dental arch or moving the teeth backward, it may be possible to avoid extracting healthy teeth.

However, when the protrusion is severe or there is a significant lack of space, forcing treatment without extraction may make the profile around the mouth appear even more prominent or place stress on the gums.

Just as forcing yourself into clothes that are too small can damage the clothes and make your body uncomfortable, forcing treatment without extraction when space is insufficient can place a burden on both the teeth and the surrounding tissues.

How uncomfortable is extraction and the period afterward?

Tooth extraction is performed under anesthesia, so many people do not experience significant pain during the procedure.

However, swelling and pain may occur for several days after the anesthesia wears off, depending on the individual. They are generally known to gradually subside over several days to about a week.

Immediately after extraction, focus on controlling bleeding with gauze, and avoid using straws, smoking, and irritating foods for several days to support recovery.

Just as the speed at which the same wound heals can vary depending on how you spend the first few days, care during the days after extraction can affect recovery as much as the procedure itself.

What side effects can occur after extraction?

Immediately after extraction, swelling, pain, and mild bruising may occur depending on the individual.

In rare cases, a dry socket, also known as alveolar osteitis, may develop if the blood clot becomes dislodged, causing increased pain. Temporary sensory changes may also occur if the extraction site is close to a nearby nerve.

During the process of closing the space after extraction, root resorption or temporary gum recession may also occur in some individuals.

The emotional impact of experiencing the same symptom can be different when you begin treatment knowing about its possibility versus when it occurs unexpectedly.

What happens to the extraction site during orthodontic treatment?

The extraction site does not remain empty permanently.

Over time, the orthodontic appliances gradually move the surrounding teeth into the space to fill it.

The time required to close the space and the stability of the result may vary depending on how the speed and sequence of tooth movement are planned. Therefore, it is important to design this aspect carefully during the initial planning stage.

After tooth movement is complete, maintenance care, including wearing retainers, must continue to help prevent the teeth from shifting again over time.

Just as moving one puzzle piece requires the surrounding pieces to be repositioned, movement after extraction is not an issue involving only that one space. It is a process of fitting the entire arrangement together.
