1. Why Is a Sinus Lift Needed Only for Upper Molar Implants?

Many people misunderstand this procedure as a type of nose surgery because of the term “maxillary sinus,” or assume that it is an extremely complicated and dangerous process.

To easily understand what a sinus lift means, we first need to look at the structure of the face.

The “maxillary sinus” is an empty space located on both sides of the nose, inside the cheekbones.

The roots of the upper molars are located directly beneath this space.

However, if a tooth has been missing for a long time or there has been severe gum disease, the jawbone may be resorbed, making the bone between the maxillary sinus and the gums very thin.

This is similar to the ceiling of the second floor of an apartment building becoming so thin that it is almost touching the floor of the third floor.

What should be done to place a strong pillar—an implant—in this situation?

Yes. The ceiling needs to be reinforced to secure enough thickness for the pillar to be firmly fixed in place.

A sinus lift is a procedure in which the lower membrane of the maxillary sinus is carefully lifted so that an implant can be placed in the thinned upper jawbone, and bone-grafting material is inserted into the space. This creates sufficient bone height and thickness for implant placement.

It is not necessary for every upper-jaw implant. Whether it is needed is determined after precisely evaluating the amount of remaining bone and the condition of the maxillary sinus through a 3D CT scan.

2. Sinus Lift Pain: Is It Severe Enough to Be Unbearable?

Many people worry about extreme pain because it is described as “a procedure involving bone grafting.”

Some swelling and discomfort may, of course, occur after surgery.

However, contrary to what many people expect, it is generally closer to a dull, heavy sensation than to sharp pain.

The level of pain after a sinus lift varies depending on a person’s condition and the extent of the surgery. In most cases, recovery is possible without significant disruption to daily life if you take the prescribed medication properly and follow the provided precautions.

• 2–3 days after surgery: Swelling may be at its most severe. Applying an ice pack can help reduce swelling.

• During recovery: Bruising may occur, but it will naturally fade over time.

• Precautions: It is best to avoid actions that increase pressure inside the maxillary sinus, such as blowing your nose forcefully, using a straw, or coughing.

If severe pain that cannot be controlled with the prescribed medication continues, or if unusual symptoms appear, it is important to visit your dentist promptly for an evaluation.

3. Sinus Lift Side Effects: What Symptoms Should You Watch For?

Unexpected situations can occur with any medical procedure.

Possible side effects of a sinus lift include perforation—or tearing—of the sinus membrane, infection, and worsening sinusitis.

The sinus membrane is particularly thin, at approximately 0.1 mm, so it may tear during surgery when the practitioner has limited experience.

This may cause inflammation or prevent the grafted bone from remaining in its proper position.

Therefore, for a successful sinus lift, it is a good idea to carefully check the following:

• Extensive clinical experience: Check whether the provider has a deep understanding of anatomical structures and experience with a wide range of cases. Delicate and precise techniques can reduce the possibility of damaging the sinus membrane.

• Advanced diagnostic equipment: It is important to determine whether the clinic accurately evaluates each patient’s sinus shape, membrane thickness, and amount of remaining bone using advanced equipment such as 3D CT, and uses this information to plan the surgery.

• Systematic follow-up care: Look for a clinic with a system that can respond promptly to problems that may occur after surgery and regularly monitor the condition until the grafted bone becomes well established.
