Radiesse is a medication that promotes collagen production. It not only adds volume, but also directly stimulates fibroblasts in the skin to induce the production of collagen and elastin.

When people say collagen injections, yes—that’s right, they’re Sculptra and Juvelook.
Sculptra and Juvelook activate collagen production by inducing a mild inflammatory response in the skin. In this process, the skin recovers on its own, elasticity increases, and wrinkles are softened.
In contrast, Radiesse directly stimulates fibroblasts without an inflammatory response to induce collagen production. In other words, it improves elasticity naturally while placing less burden on the skin.
Because neck skin is thin and sensitive, excessive inflammation can raise the risk of side effects such as nodules (hard lumps). By contrast, because Radiesse induces collagen production without an inflammatory response, it can be safely applied even to thin skin.

Therefore, it is important to adjust the dilution ratio according to the patient’s skin condition—such as the treatment area and degree of aging—and the desired outcome.
For example, when improving signs of aging or adding moderate volume, it is diluted to about 1x. When improving specific facial wrinkles such as around the mouth or nasolabial folds, diluting it 2x is recommended. In addition, for a more natural volume effect or for younger patients, it may be diluted 3x.
Then what about treating neck wrinkles?
Usually, it is diluted 3x for treatment.
Because neck skin is thin and wrinkles are spread over a wide area, using too concentrated a formulation can cause it to gather in specific spots. That’s why it should be mixed with saline and lidocaine (I’ll explain the importance of lidocaine dilution in the next section) and injected gently.
In addition to neck wrinkles, Radiesse is also effective for nasolabial folds, lines around the mouth, smile lines, and the outer cheeks.

At this time, as mentioned earlier, the procedure is performed with dilution of about 2x.
However! There is no fixed rule that says, “nasolabial folds must always be diluted 2x.” The dilution ratio can vary depending on the patient’s age (degree of aging) and the level of improvement desired. The dilution ratios mentioned above should be used only as rough guidelines.
Ultimately, what matters is precisely adjusting the dilution ratio to match the patient’s skin condition and desired results.
Uniform drug injection technique
The outcome of Radiesse depends completely on how it is injected. Especially in areas like the neck, where the skin is thin and there are many blood vessels, it is very important that the medication spreads evenly.
First, with a regular needle, there is a high chance that the medication will collect in one place. It can be injected intensively into a specific point.
Also, because the tip of the needle is sharp, there is a risk of injuring blood vessels or nerves, and the pain tends to be greater.

A treatment method that addresses the limitations of regular needles is the cannula.
A cannula is a device with a rounded, soft tip instead of a pointed needle tip, allowing the medication to be injected while passing smoothly through tissue without cutting or puncturing it.
Because of this, pain is significantly reduced, and the risk of directly damaging blood vessels or nerves is almost nonexistent.
Most importantly, because a cannula can be moved in a controlled direction while injecting the medication, it allows the medication to be distributed evenly over a wide area.
In other words, neck skin is much thinner and has more blood vessels than the face, so using a regular needle carries a greater risk of bruising or medication clumping. A cannula, on the other hand, can move smoothly and distribute the medication widely and evenly, making it far more natural, uniform, and likely to produce satisfying results.
Therefore, when getting a Radiesse neck wrinkle procedure, it is important to check whether a cannula is used.
And one more thing,
If lidocaine is diluted together for the procedure, pain can be reduced even further. So be sure to check whether lidocaine is included in the dilution.
