Here, not all thyroid cancers are included; prescribing is difficult only when there is a family history of medullary thyroid carcinoma.
Because GLP-1 receptor agonists are related to thyroid C cells (cells where GLP-1 receptors exist), they may increase the risk of medullary thyroid carcinoma (MTC), which develops in these C cells. However, common thyroid cancers such as papillary carcinoma or follicular carcinoma are not related to C cells, so they are not associated with Wegovy use.
In other words, it is not “all thyroid cancers” that are risky; only “medullary thyroid carcinoma” is related to Wegovy use, so this distinction must be carefully considered.
- History of acute pancreatitis

When prescribing Wegovy, checking whether there is a family history of medullary thyroid carcinoma, as mentioned above, is just as important as checking for pancreatitis.
If you currently have pancreatitis, Wegovy cannot be prescribed under any circumstances.
Wegovy is a GLP-1 receptor agonist that acts on pancreatic beta cells to lower blood sugar. The problem is that GLP-1 receptors also exist in the pancreas, so this medication may worsen inflammation of the pancreas.
And even if you had pancreatitis in the past, you must decide carefully after consulting a medical professional.
Of course, it cannot be stated definitively that GLP-1 receptor agonists directly cause pancreatitis. Existing risk factors such as gallstones, hyperlipidemia, and alcohol intake can also have an effect.
However, since the possibility of causing pancreatitis cannot be completely ruled out, the U.S. FDA and the European EMA recommend that GLP-1 agonists may increase the risk of pancreatitis, so patients who previously had acute pancreatitis should use them cautiously.
Both current pancreatitis status and past medical history must be reviewed.
- History of diabetes and hypoglycemia
GLP-1 is a hormone naturally secreted in the body after eating food. It stimulates pancreatic beta cells to increase insulin secretion and, at the same time, suppresses glucagon secretion (the hormone that raises blood sugar).
There is an important point here.
Wegovy promotes insulin secretion only when blood sugar rises after eating, so under normal circumstances the risk of hypoglycemia is not high. On the other hand, when taken with other glucose-lowering drugs such as insulin or sulfonylureas, the risk of hypoglycemia can increase.
- If you are already using insulin or sulfonylureas
For patients already using insulin injections or taking sulfonylureas, Wegovy administration may cause excessive insulin secretion and increase the risk of hypoglycemia. That is why, when starting Wegovy, the dose of existing glucose-lowering medication often needs to be adjusted.

- If insulin resistance is severe (difference between early diabetes and advanced diabetes)
Among patients with type 2 diabetes, in the early stages insulin resistance is the main problem, so increasing insulin secretion may be helpful. However, if diabetes is significantly advanced (long history of diabetes, reduced pancreatic function), using Wegovy may put the pancreas at risk of overworking.
In other words, Wegovy’s effect of promoting insulin secretion is not automatically bad for patients with diabetes, but it should be used cautiously depending on whether the patient is already taking medication and on the condition of pancreatic function. If you plan to use Wegovy, it is important to consult a medical professional and continuously monitor blood sugar changes.
When prescribing Wegovy, it is essential to check for a family history of thyroid cancer, pancreatitis, diabetes, and hypoglycemia.

In addition to these medical histories, whether you have experienced yo-yo weight regain in the past is also something that must be checked when prescribing Wegovy.
Yo-yo effect? Yes, that’s right.
For people who have repeatedly tried dieting in the past and often experienced yo-yo weight regain, it may be difficult to lose weight with Wegovy alone.
Why prior yo-yo weight regain matters when prescribing Wegovy
- Lower basal metabolic rate
People who have experienced repeated dieting and yo-yo regain are more likely to have a lower basal metabolic rate.
When dieting, body weight often decreases along with muscle mass, which slows metabolism and can shift the body into a “conservation mode” that saves energy even with fewer calories.
Wegovy reduces calorie intake by lowering appetite, but if the basal metabolic rate is already low, weight loss may be slower even with the same calorie reduction effect.
- The body’s “homeostasis” mechanism that tries to maintain weight
The body has homeostasis that tries to keep the current weight stable.
For people who have repeatedly regained weight, when their weight decreases, the body tends to send stronger signals to store fat. In particular, the balance between appetite-suppressing hormones and appetite-stimulating hormones can become disrupted, making it easier to feel hungry during weight loss and increasing the body’s tendency to store fat.

- Psychological factors and compensatory behavior
People who have repeatedly dieted often have a high psychological dependence on food or have difficulty controlling meals in stressful situations. Wegovy helps regulate appetite, but emotional eating habits (stress eating, late-night snacking, and similar patterns) cannot be completely resolved with medication alone.
Therefore, behavior change and cognitive control must happen together for weight loss to be maintained in the long term.
