In short, the design process is about fully understanding the style the patient prefers and planning the lips so that those needs blend naturally into their actual facial structure.
That said, the patient’s requests are not followed 100% unconditionally.
The doctor must consider the face shape, the distance between features, and the angles of the philtrum and the corners of the mouth in order to suggest the most harmonious and natural design direction.
In short, the procedure should be carried out in a direction that suits the patient’s face well, within the scope of the patient’s needs and medical judgment.

If a patient has a long and narrow face, adding volume to the lower lip can help adjust the facial proportions and create a more stable balance. On the other hand, if the face is wide or round, it may be more harmonious to avoid overfilling and instead emphasize the lip’s horizontal length and contour to create a more refined impression.
The goal is to suggest a better direction within medical judgment while also meeting the patient’s needs.
In addition, lip design should consider not only the lips themselves but also the corners of the mouth, the philtrum, and the left-right symmetry of the face in order to be complete.

Rather than vaguely saying, "I want the corners of my mouth lifted a little," it’s much more intuitive to show a photo and say, "I want something like this!"
However, it is realistically difficult to create the exact same lips as if doing a copy-and-paste.
We use the mood and feeling of the photo as a reference and then adjust the shape so it best suits your face.
As I mentioned earlier, lips do not exist on their own. They are affected by the face shape, the length of the philtrum, the direction of the mouth corners, and even how much of the teeth are visible. So rather than saying, "Please make it exactly the same," it’s better to think in terms of, "Please adjust it beautifully in this style."
The same goes for the idol lip styles that are popular these days. It’s a style where the lower lip is round like a peanut and the center volume is slightly emphasized, but because the look that suits each face shape is different, the same design can come across a little differently from person to person.
