You can easily come across comments like these.
I also receive questions like this very often.

To be honest, I understand why it can feel that way.
Right after the procedure, you may be satisfied, thinking, "Wow, it really lifted," but a few months later you might wonder, "Huh? Is this right?"
Actually, that reaction is completely natural.
That’s because if you don’t know about the real changes happening under the skin, you end up judging only by the immediate tightening you see right after the procedure.
But in reality, Ulthera’s effects do not disappear after 3 to 4 months.
Even after 3 to 4 months, changes continue under the skin, and the actual effects are maintained for about a year.
So why does it feel like the effects disappear after just 3 to 4 months?

First, let me explain how Ulthera works. You need to understand this in order to know why it can feel like the effects are short-lived.
It delivers high-intensity focused ultrasound energy to the dermis and SMAS (the fascial layer), creating thermal coagulation points. At that moment, the temperature rises to around 60–70°C, and the existing collagen fibers contract immediately.
Simply put, it’s like heating a squid, which makes it shrink tightly.
This is the immediate effect that makes you say, "Wow!" right after the procedure.

But that’s not the end.
In fact, the real effect of Ulthera starts from here.
As the tissue that received the heat stimulus enters healing mode, it begins producing new collagen on its own.
At first, Type III collagen is rapidly formed, and then it changes into Type I collagen, creating a dense network. The newly grown collagen becomes an "invisible support structure" that helps prevent skin sagging.

In the end, the reason the effect feels short is because of the immediate contraction effect right after the lift.
In reality, long-term changes are taking place over 6 months to a year as new collagen grows within the skin.
In other words, Ulthera’s effects continue even after 3 to 4 months.

This is the period when you tend to feel the most dramatic changes.
As some of the swelling that occurred immediately after the procedure subsides, the contracted appearance becomes more visible, and at the same time, new collagen production is actively taking place in the skin that received the heat stimulus.

These two effects overlap, making the skin feel the tightest during the 1-3 month period.
Especially around 3 months, as newly formed collagen accumulates to some extent, you can most clearly feel that the skin has become firmer.
That’s why many people think of this period as the "best effect".

- 3-6 months
As the strong tightening you felt during the 1-3 month period eases a little, you may wonder, "Huh? Is the effect disappearing?"
But in reality, the newly formed collagen is in the process of settling into place within the skin.
As the collagen becomes more stable, the tightening may look less obvious on the surface, but this is actually the period when the internal structure of the skin is becoming stronger.

This is the point when collagen remodeling is histologically most complete.
From this point on, the skin maximizes its support to prevent sagging from returning.
In the end, the reason many people feel that Ulthera’s effects are short is the dramatic contraction seen at 3 months.
But in reality, the true effects of Ulthera are completed starting after 6 months.
Still, it’s understandable to feel disappointed when that dramatic lifting effect at 3 months fades.
Even if meaningful changes are happening under the skin, combining procedures that can help the lifted effect last longer can be a good alternative.

It is a method of adding filler while the skin is lifted by Ulthera.
If hyaluronic acid filler is injected in this lifted state, it creates a physical support beneath the tightened tissue, helping keep the skin from settling back down.
Simply put, if Ulthera plays the role of lifting, filler plays the role of supporting that position, so the visibly tightened effect lasts much longer.

- Thermage
While Ulthera concentrates ultrasound energy deep into the facial fascia layer to instantly tighten collagen fibers, Thermage spreads heat evenly throughout the dermis, helping new collagen form and the surrounding tissue become denser.
When the two procedures are performed together, Thermage fills in the facial contours that have been firmly lifted from within by Ulthera, from the outer layer to the middle layer of the skin, allowing both deep lifting and broad improvement in elasticity at the same time.
As a result, the immediate tightening effect after Ulthera can last longer.
