That is, extraction orthodontics & non-extraction orthodontics.
- Cases where extraction orthodontics is needed
First, let’s look at when extraction orthodontics is necessary.
(1) When the teeth protrude
(2) When there is severe crowding
(3) When there is a tooth with severe decay
(4) When the left and right dentition are asymmetrical


When the teeth protrude or when there is severe crowding,
a space must be secured by extracting teeth so that the teeth can move
enough left/right/front/back,
which makes it possible to improve protrusion and tooth alignment.
Therefore, extraction orthodontics is recommended.
Also,
when there is severe decay that requires extraction,
or when the left-right dentition asymmetry is severe,
it is also recommended to proceed with orthodontic treatment after extraction.
- Which teeth are extracted?
Generally, when counting from the front incisors in order,
the 4th teeth are the most preferred for extraction.

Teeth #4 and #5 are both premolars with the same function,
so compared with front teeth or molars,
they are considered teeth that play a lesser role.
Front teeth are used to cut food,
and molars are used to chew food thoroughly,
but premolars relatively do not perform such functions very much.
Meanwhile, if tooth #3 or #5 has severe decay,
the severely decayed tooth may be extracted,
and the opposite symmetrical tooth may also be extracted.
- Cases where non-extraction orthodontics is performed
If extraction is not absolutely necessary,
it is better to consider non-extraction orthodontics first.
For example,
▶️ If there are many spaces between the teeth
▶️ If the teeth are congenitally small
If extraction orthodontics is performed, the teeth may be pulled in too much
and may look like a “sunken mouth,”
so non-extraction orthodontics is preferable.
Also,
▶️ In cases of deep bite
(when the upper teeth cover the lower teeth too much)
if extraction orthodontics is performed, the deep bite may worsen,
so non-extraction treatment is preferred.

If you are correcting protruding teeth,
a protrusion of about 2 to 3 mm can be improved without extraction
through arch expansion and backward movement of the molars.
- Cases where extraction is recommended,
but non-extraction is chosen

or irregular tooth alignment,
but
if the patient (or guardian) strongly prefers non-extraction
due to resistance or fear of extraction,
treatment may proceed without extraction.
When orthodontic treatment is done this way,
the crooked teeth become evenly aligned,
but because there is not enough space,
the teeth are arranged more neatly overall,
and protrusion toward the front may appear.
In fact,
many people initially had non-extraction orthodontic treatment elsewhere,

Even from the dentist’s perspective, there are cases where it is difficult to decide
whether extraction should be performed or treatment should proceed without extraction.
If the degree of protrusion, tooth size,
and arch size place the case near the boundary between extraction and non-extraction,
it may be good to proceed without extraction for about 6 months first,
observe the progress,
and then make a re-diagnosis
and decide whether extraction is needed.
From the patient’s perspective as well, checking the changes in your face after 6 months
and discussing with the medical team which direction would be better
can lead to a wiser decision.
