Today, I’d like to begin with a question patients ask very often.
My teeth are crooked—aren’t they just a cosmetic issue?
This is one of the questions I’ve heard many times over the 25 years I’ve been practicing orthodontics.
But to get straight to the point, it is not simply a matter of whether they look pretty. Behind the visible problem of misaligned teeth, there are much more complex and important reasons.
Today, I’d like to explain those complex and important reasons.
Uneven teeth mean there is not enough space

What we commonly call protruding teeth is not just a matter of alignment.
In fact, a number of complex factors influence it, such as a lack of space in the upper and lower jawbones, the position of the tongue, and the habit of breathing through the mouth.
For example, if a child had the habit of breathing through the mouth during growth, the tongue position would drop lower, and the upper jaw would not widen properly.
As a result, there is not enough space for the teeth to come in, and they naturally become crooked.
▶ In other words, please remember that problems with tooth alignment are not just issues with the teeth themselves, but are connected to the overall structure of the jaw and oral cavity.
Uneven teeth also affect chewing function and the temporomandibular joint

When teeth are crooked, it is not only uncomfortable to look at, but functional problems also arise.
Chewing function decreases, placing excessive force on certain teeth only, and this imbalance can lead to temporomandibular joint pain, headaches, and in severe cases, some adult patients even complain of neck and shoulder pain.
※ The purpose of orthodontic treatment is not simply to create a pretty arrangement.
The real core of orthodontics is to create a normal bite—that is, a structure that allows proper chewing!
Not knowing this and simply aiming for a pretty set of teeth can actually be a little risky.
Extraction decisions, wire selection, and everything else are all determined by the current state of the teeth

If you think orthodontic treatment is just a matter of putting on a wire, that is a big mistake!!
Depending on how much the teeth are rotated, how far inward they are positioned, and whether the left and right sides are symmetrical, completely different treatment plans are needed.
For some people, extraction is essential, but in other cases, better results may come from not extracting teeth.
Even now, when I look at each patient’s alignment, I diagnose and plan with the feeling that I am solving a new puzzle every time.
That is how different each person’s oral structure is, and there is not just one correct answer!

Yes, of course, that may be possible.
But the issue is that saying things are fine now does not mean they will stay fine in the future.
From the mid-30s onward, food tends to get stuck more easily between crooked teeth, and gum health begins to worsen gradually.
And there is a common thing people who start orthodontic treatment after their 40s say.
“I wish I had done it earlier.”
Problems with tooth alignment are not solved by time. In fact, they tend to get worse as time goes on.
