If the lower jaw (underbite) protrudes, it can cause several inconveniences.
The first thing people notice is that the jaw looks long.
However, there are two cases here, and the treatment approach can differ slightly.
- When the lower jaw itself is very long
(underbite)

The first case is when the lower jaw itself is very long.
This is commonly called an underbite.
- When the jaw is somewhat long,
and the lower front teeth protrude outward

The second case is when the lower jaw is somewhat long,
but in addition, the lower front teeth protrude outward,
causing the upper and lower front teeth to bite in reverse.
In these cases, along with the lower jaw length,
the protrusion of the lower lip is also more noticeable,
and the chin tip may actually look like a receding chin.
These two cases differ
in terms of treatment method.
Treatment Method #1 for a Case Where the Lower Jaw Itself Is Very Long
First, if the lower jaw is long and the lower front teeth are tilted backward,
it is difficult to solve with orthodontics alone.
What the patient wants is
‘to reduce the length of the lower jaw,’
but orthodontic treatment cannot physically shorten the length of the jawbone.
In such cases, surgery that actually cuts the lower jaw bone to reduce its length,
that is, double-jaw surgery (jawbone resection), is needed.
Only then can the appearance improve due to the jaw length.
Treatment Method #2 for a Case Where the Jaw Is Somewhat Long and the Lower Front Teeth Protrude Outward
The second case is different.
These patients can also achieve a greater degree of appearance improvement with double-jaw surgery,
but
there is still something to gain from orthodontic treatment alone.
When the reverse bite is corrected, the relationship between the upper and lower front teeth returns to normal,
reducing lower lip protrusion,
and producing a side-profile improvement effect.
The chin line also becomes more defined,
improving the appearance of a chin that looks recessed.

In fact, after orthodontic treatment,
many people say, “My jaw feels shorter.”
In this case, even without surgery,
non-surgical orthodontic treatment can improve both appearance and function.
To summarize,

However, many people simply think,
“Long jaw = surgery is necessary,”

But in reality, there are many more people who fall into the second type,
where protrusion and the chin line can be improved with orthodontic treatment alone,
without double-jaw surgery.
