Have you heard that you should not leave an empty space when a tooth is unavoidably lost?
Today, we would like to explain this case.
The patient needed an implant, but there was not enough space, so orthodontic treatment was first performed to create room for implant placement, followed by surgery.
01. Consultation

This patient visited us after losing two lower molars.
Because the teeth had been missing for a long time, the nearby molars had tilted to the side.
In this situation, there was not enough space to place the implants, so it would have been difficult to proceed immediately. We explained that orthodontic treatment should be performed first to create space, followed by implant placement.
Fortunately, the patient understood the treatment plan well and decided to proceed with both orthodontic treatment and implants.

02. Wisdom Tooth Extraction

We decided to extract the wisdom teeth first.
The areas marked in the image indicate a total of three teeth.
Extracting the wisdom teeth creates space at the back, making it easier to move the teeth during orthodontic treatment.
Next, orthodontic brackets were attached.
Using the space that had been created, we guided the teeth into their proper positions.
03. Beginning Orthodontic Treatment and Implant Placement

Approximately x months after starting orthodontic treatment, the first implant surgery was performed.
One implant was successfully placed in each of the left and right lower molar areas.
Because this patient had insufficient jawbone, bone grafting was performed at the same time, and we decided to place the implant crowns three months later.

Fortunately, the implants were placed at a stable depth and in the correct direction.
We could also confirm that more jawbone had formed compared with immediately after surgery.
Since the jawbone had formed well, we proceeded with taking impressions for crown fabrication.
04. Completion of Orthodontic Treatment and Implant Placement

The crown restoration was successfully completed three months later.
Fortunately, the patient experienced no discomfort with the prosthesis, and we carefully checked whether food was getting trapped.
Orthodontic treatment was completed three months after this.
Because our dental clinic provides both orthodontic and implant treatment, the patient was very satisfied that treatment could be carried out efficiently in terms of both cost and time.

In this patient's case, we initially approached treatment as orthodontic treatment to prepare for implants. However, the misaligned front teeth and overall alignment were also corrected, which seems to have led to even greater satisfaction.
When both orthodontic and implant treatment are needed, as in this case, you should allow for a longer treatment period and carefully review the treatment plan.
