| A Story About Undergoing an Implant Procedure with Sinus Surgery |
Hello, this is Seoul Centum Dental Clinic.
As I conduct implant consultations every day,
I often think I should write more blog posts about
sinus lift surgery.
These days, people often come in thinking about
implant brands and bone grafting,
but I feel that information about the maxillary sinus is lacking.
Seeing is believing,
today I will explain it simply
by showing you an actual case.

Consultation Details
This patient came in from the start for an implant consultation,
and fortunately, it was not a difficult case.
However... this patient said that during dental treatment,
they had a strong gag reflex,
so it had been difficult to receive treatment in daily life.
In such cases, I make a note in the treatment chart in advance
and pay extra attention,
so I remember that we were able to finish everything well,
from diagnosis to treatment.

The lower molars had already been lost,
so implant treatment was needed,
but the patient wanted to proceed with the upper side first,
so we planned implant placement after extracting the upper teeth.
In particular, before placement and sinus surgery,
a CT scan was taken to check the exact position of the maxillary sinus
and the implant placement site.

We planned to place implants in the upper left premolar and molar areas,
but this was a case with insufficient placement space.
Therefore, we proceeded with implant placement with sinus surgery
and bone grafting.
What is a vertical sinus lift?
After making a small opening at the site where the fixture,
which corresponds to the tooth root, will be placed,
the membrane surrounding the air-filled space called the maxillary sinus
is carefully lifted to create space for placement.
It is called "vertical" because the membrane is lifted vertically.

We allowed time for enough gum bone to form,
and the patient returned about 3 months later.
Fortunately, it was forming well,
so we decided to proceed with making the prosthesis.
If you look at the X-ray,
you can see that the fixture has integrated well with the bone
and has been placed stably.
Here, we placed a one-piece fixture that does not require a second surgery
and immediately proceeded with prosthesis fabrication.

The prosthesis was also stably fitted.
Of course, at first it was temporarily attached,
and there was a period of use to see how it worked.
Considering how severe the gag reflex was,
the implant placement with sinus surgery
was all completed successfully.
For people like this, using a one-piece fixture that does not require a second surgery
and reducing the number of treatment visits is very helpful.

The prosthesis was attached with temporary cement,
used for about 10 days,
and then permanently bonded to finish the treatment.
The lower side also needed treatment,
but because it was difficult for the patient to receive dental care,
they decided to return later for that part.

A sinus surgery is not necessarily difficult
because it is performed at the same time as implant placement,
so it is relatively less burdensome.
However, you may worry about the cost and side effects,
so next time I will explain those parts
in an easy and honest way as well.
