Most people
think,
'If you get root canal treatment, you absolutely have to put on a crown.'
Of course, that is true in textbook terms.
In fact, that is what I learned in school too....^^;
But if it is a small molar that does not bear strong chewing forces,
you do not necessarily have to cover it with a full crown!

Today, I want to show you a case where a tooth that had been broken more than halfway by decay was treated with root canal therapy,
but finished not with a full-coverage crown,
rather with an overlay crown.

There were already 'signs' from our first meeting.

When the patient first came in, we helped reattach a gold inlay that had been placed at another clinic after it came off.
Even when patients only want treatment for the area that feels uncomfortable,
I perform an overall examination.

As shown in the photo above, the decay was progressing,
and although I recommended treatment,
the patient went home saying they would do it in six months.
Time passed, and nine months later,
the molar had completely fractured,
and the patient returned to the clinic.

When we checked with a panorama, intraoral photos, and probing examination,
the damage was larger than expected,
and the decay had progressed deeply.

After cleaning out the contents, it was already contaminated in the pulp.
Root canal treatment came first..
Wait, why does this keep happening at gold inlay sites?

In the past, gold was loved for being 'precise,' but because it is retained by cementation rather than true adhesion,
small gaps form over time, and bacteria get into those gaps.
That is why secondary decay develops,
and the thinned tooth can crack progressively like a wedge under chewing force.
One reason gold inlays are not preferred these days is not only because gold prices have risen,
but also because they can cause teeth to fracture.
That is why I prefer treatments that bond well without breaking the tooth.
It is a design for 'minimum removal, maximum preservation.'

The plan was as follows.
➡️ We would reinforce the remaining tooth structure so it could support itself.
A resin wall (think of it as a protective barrier^^) would be built around the tooth to prevent leakage of medication or reinfection of the canal.
➡️ After that, under a rubber dam (moisture control), we would completely clear out the infection with root canal treatment.
➡️ Instead of shaving down the entire tooth and covering it with a crown, we decided to bond a conservative prosthetic restoration, an E-max overlay
(Partial Veneer/Three-Quarter Crown),
which preserves the walls that need to be kept and covers only the weak walls.

We prevented a small failure from becoming a big one.

We started with root canal treatment.
A rubber dam was used to thoroughly block saliva and blood,
then the infected pulp tissue was removed,
and the canals were enlarged and shaped to prepare for cleaning and filling.
We repeated length measurements and performed sealing^^
One important point here is that
root canal treatment is not the end.

The weakened core needs to be rebuilt for long-term durability.
So we laid a foundation with a UHMWPE fiber post,
and filled the inside with everX (FRC-reinforced resin)
to suppress crack propagation.
It sounds a bit difficult, right?ㅎㅎ
To put it simply,
inside, it is like rebar and concrete,
and outside, it is wrapped in a shell with elasticity similar to natural tooth structure.

For the prosthetic, we chose an E-max overlay.
Instead of a full crown that shaves down the entire tooth,
this method selectively covers only the necessary surfaces.

During preparation, healthy walls were left in place to secure a ferrule
(a reinforced rim that resists leverage),
and after checking the chewing height and mobility with a temporary tooth,
the final restoration was fully bonded with resin cement.
Throughout this entire process, I never gave up on details such as rubber dam isolation, surface pretreatment,
and curing time.
In conservative treatment, detail is durability.
After permanent bonding, the bite was adjusted carefully and the case was finished.
“So there was a way other than an implant.”

The patient's first reaction was,
"I thought the tooth would have to be extracted and replaced with an implant,
but there was a method like this."

The pain subsided, and the fractured area became strong as if it were one piece through reinforcement and bonding.
The shape was not excessively thick,
and the function was not excessively lowered,
so the balance of aesthetics and strength was maintained.
Most importantly,
we preserved the original tooth.

If possible, I keep looking for ways to remove less tooth structure, extract less, and help teeth last longer.
Even though implants have improved a lot, they still cannot completely replace the biological advantages of natural teeth.

That is why I persistently focus on words like rubber dam, moisture control, adhesion, reinforcement, and partial prosthetics.
They may look cumbersome and take more time, but the result is different.
Rather than doing it roughly in one go, doing it properly from the start ultimately protects the patient's time, cost,
and the tooth's lifespan together^^

Every day in the clinic, I ask myself the same question.
"How far can we preserve this tooth?"
Even with a molar that has broken completely, like today's case,
I will keep looking for a way to preserve it to the very end.
And along that path, I will not give up on the details.
As in today's case,
there are truly many ways to save a tooth.
Even if you have been told extraction is necessary, do not give up easily😊
