3 years ago, I had root canal treatment at another clinic
and was told it needed to be crowned.
When I drink carbonated beverages, it also feels sensitive,
so it really does seem like I need root canal treatment..
But I’m in my 20s,
and I don’t want my tooth shaved down this early,
so I’d like to ask whether there are any other options
and hear other doctors’ opinions.

I’ll explain in detail how, despite having symptoms, we were able to treat it with resin without root canal treatment!

The front tooth cavity was deep, but not to the point where root canal treatment or a crown was immediately necessary.

Even when treating it,
it’s really important to reduce unnecessary removal
as much as possible.
Because once treatment starts like that,
there’s no choice but to do larger and larger procedures.
So whenever possible, I prefer to use resin, which requires the least removal.

After checking the cavity, it was deep, but I judged that it did not require root canal treatment.
So I carefully removed it and decided to finish with resin.

After examining the entire mouth, I found a cavity in the farthest molar on the left as well.
So we decided to treat the molar (#27) with resin too.

Front-tooth resin treatment is often thought of as a “simple procedure,” but in fact it is a very delicate job that requires a lot of work^^;

After placing a rubber dam to isolate the tooth from moisture, we had to cure it layer by layer with light and shape it so that it blended naturally with the neighboring teeth.

The molar was also rebuilt naturally after removing the cavity.

Two weeks later, the patient returned for a follow-up check.

Even when checked with Qray, there was no cavity visible, and we could confirm that it had been completed neatly.
That day, I checked the entire mouth once again and noticed a tiny crack in the lower right molar.
They went home after scheduling the next appointment.
What happened before molar treatment

3 weeks later, the patient returned, saying they wanted treatment because the resin previously placed in the lower left molar had worn down.

When I actually checked, the previously filled material had discolored and the margins had come off, and there were areas where new decay had formed, so it needed to be replaced.

They only wanted the lower right molar (#37), so I treated just that one.

I proceeded by minimally removing only the cavity using Qray.

After that, I filled it neatly with resin.
When I asked about the treatment plan for the other molar and the cracked molar, the patient went home saying, “Your treatment seems good... but it’s too expensive...”
A month later, they contacted us again.
They said they had been treated at another clinic, but the color felt awkward, the shape was unsatisfying, and above all, it broke and hurt not long after.

There was a thick layer of a white hardened material (presumed to be ZPC) underneath the existing material.

In simple terms, the resin was not attached directly to the tooth; it was attached on top of a “cement-like material.”
This structure means that no matter how well the resin is done,
it will inevitably chip or come off...
Resin becomes strong only when it bonds directly to the tooth.

So we removed the previously placed material as cleanly as possible and re-prepared the base.
Then we rebuilt the structure so the resin would bond firmly directly to the tooth, and redesigned the shape so that chewing forces would be evenly distributed.

We treated one tooth on the lower right (46), then one on the lower left (36), and finally one on the upper left (26), spacing out the dates so they could receive insurance coverage one by one.

Our fees may feel high.
And resin fees are higher than inlay fees, so patients sometimes have questions about that too.
