The Secret to Naturally Filling Black Triangles

Doctor, because the gap between my front teeth keeps showing,

I’m self-conscious every time I smile.

The patient I’m introducing today is a 27-year-old office worker. Their workplace is near the clinic, so they stopped by while passing through and gathered the courage to come in.

After the treatment, I posted it on Instagram Stories, and many dentists hit the “like” button. The patient also really liked the result, so I’d like to introduce this case in more detail.^^

The trendy Bio Clear these days, but here’s why I don’t use it often.

There is a method called “Bio Clear” for filling gaps between the front teeth.

These days, it’s been introduced on YouTube as a way to close front-tooth gaps, so some people come in saying, “Please do Bio Clear for me~”

It’s excellent for closing spaces. But it also has drawbacks.

The boundary between the tooth and the gum is not naturally recreated.

To put it simply, it should taper down sharply to look pretty, but the line of the front tooth ends up looking spread out, giving the impression that the front teeth look a bit puffy.

So in cases where the front teeth are spaced apart, I use a different method more often.

I mainly use Style Italiano’s

UNICA PROXIMAL.

The method I use most often is “UNICA PROXIMAL,” created by the resin study group Style Italiano.

This can create a beautiful, natural shape for the front teeth without any of the drawbacks seen with Bio Clear.

Instead, it has one characteristic.

Rather than tightly sealing the space between the front teeth, a very thin, natural-looking gap may remain.

Then people usually ask, “Doctor, doesn’t that mean the gap between the teeth is still visible?”

This was also the case with this patient, but when using Unica Proximal, a very narrow gap may still be visible immediately after treatment.

But there aren’t any patients who look at that gap and ask for it to be filled again.

It’s such a tiny amount that it’s barely noticeable, so there’s really no need to worry. hehe

Over time, as the gums gradually recover, they naturally fill in that space.

So after 2 weeks, 1 month, or 2 months, the result often looks much more natural than it did at first.

Another advantage is that floss can pass through easily, making hygiene care simpler.

If the space is sealed too tightly, floss won’t go through, and fine debris can get trapped more easily. That makes the gums more likely to swell.

Simply put, the gums also need room to breathe.

An X-ray was taken to check the overall condition, and we confirmed that there were no other problems.

The patient wanted treatment on the same day, so we proceeded right away.

It wasn’t just the gap between the teeth; a small cavity was also found at the boundary area, so we decided to treat it at the same time.

So first, we cleaned out the weakened area on the inner side of the front teeth and the area where the cavity had started, and strengthened the inside.

We placed a rubber dam and used Unica Proximal to design the shape between the front teeth with resin and build it out.

As a finishing touch, we used a very thin polishing instrument (soflex) to smooth it out.

You can see that the gap was naturally filled.

“What if I don’t want even the tiniest gap?”

Of course, not everyone likes this kind of natural space.

Some people say, “I don’t want any space at all. Please close it completely so nothing shows.”

In those cases, I can also treat them with Bio Clear. But I always explain it clearly: “The front teeth may look a little wider, and the line may appear more rounded. Still, if you prefer it to be completely sealed, we can proceed this way.”

Because everyone’s idea of what looks good is different, I usually explain the pros and cons thoroughly and let the patient make the choice.

When filling black triangles, I think the important thing is not simply “removing the gap,” but looking at three things together: whether the shape is natural, whether there is space for the gums to grow in healthily, and whether floss can pass through easily so hygiene can be maintained.

In cases like this one, treatment that leaves a little room and respects the part that time will heal often ends up looking more natural and lasting longer.
