My friend said that because a wisdom tooth was impacted,
the nerve could be damaged...
Will I be okay?
Among the treatments patients fear most, root canal treatment and wisdom teeth are probably at the top of the list.
Many people end up getting scared after hearing about a friend’s treatment experience.

In fact, when removing an impacted wisdom tooth, there are rare cases where the nerve can be damaged.
The probability is not very high, though.
If you pay attention to just a few things, even impacted wisdom tooth extraction can be done safely.

A while ago, a woman in her 30s visited the clinic.
She said her lower left wisdom tooth had been bothering her, and as we talked, she mentioned that she thought there might also be one on the lower right.
It wasn’t extremely painful, but it kept catching her attention now and then, and her tongue kept going to it... then suddenly, unbearable pain would strike.
Most people come to the dental clinic once the pain starts.
But if you come when it hurts, the inflammation can be severe and extraction may be difficult. Preventive extraction before the pain starts can also be a good option.
Oh! Some people think I only do resin treatment... but I’m confident in wisdom tooth extraction, root canal treatment, and implants tooㅎㅎ
My first thought after looking at the X-ray was, “This won’t be easy.”

Since teeth buried under the gum are hard to see with the naked eye, we checked the exact condition through X-rays.

The upper one was straight, but both lower wisdom teeth were impacted, the roots were split into two branches, and the tips were quite curved.
Also, the distance between the root tips and the inferior alveolar nerve canal was very close.

In cases like this, the inferior alveolar nerve canal can be damaged during extraction, so extreme caution is needed.
Fortunately, there was no swelling or inflammation, so we decided to extract it that same day.
The key is not breaking the roots.
The most important thing in this case was not to break the roots.
If a root breaks... trying around to find the broken tip can actually damage the nerve canal.
So if the root breaks, sometimes the root tip is intentionally left in place.
The ideal situation is, of course, for the root not to break.

We planned to make the smallest possible incision in the gum, remove only as much bone as necessary, and divide the tooth into several pieces before removing it.
And since removing both sides at once would make eating difficult, we decided to extract the painful left side first and then do the right wisdom tooth extraction next.
After extraction, always double-check!
During the procedure, we avoided unnecessary incisions and minimized the amount of bone removed.
Fortunately, the roots did not break and came out well.

It seemed to have come out well when looking at the roots and the X-ray, but we took another X-ray for a double check.

You can see that it was removed cleanly.
Before suturing, we placed a hemostatic agent.

A hemostatic agent promotes blood clotting to help stop bleeding and supports the stable formation of the blood clot in the wound, helping recovery proceed smoothly.
It also protects the wound area, helps prevent food from getting stuck, and reduces the risk of infection caused by that.
Ten days later, the patient returned, we removed the stitches, and extracted the upper and lower wisdom teeth on the other side as well.

The right side was also removed cleanly without the roots breaking or any damage to the nerve canal.

We also placed a hemostatic agent in that area to reduce bleeding and help the wound heal stably.
Just because the extraction is over
doesn’t mean the treatment is completely finished.
Following the precautions after wisdom tooth extraction is really important.
When patients sometimes come back because the bleeding doesn’t stop well, we check and find:
✅ They did not bite down firmly on the gauze
✅ They did not keep the gauze in place for more than 2 hours
✅ They kept talking while biting on the gauze
These were the three most common reasons.
So please follow the precautions we explained to you carefully🙏🏻
The standard I consider important
in wisdom tooth extraction is!
I don’t think this is a treatment that shows how quickly the doctor can finish it.

My standard is for the patient to have less pain, less swelling, and an easier recovery.
This case was also carried out within that standard. Since impacted teeth require an incision in the gum, we reduced the incision range and minimized bone removal, for exampleㅎㅎ
If you are considering wisdom tooth extraction, I think “Please take it out quickly” is much less important than “Please remove it safely in a way that fits my condition.”
Going forward, I will continue providing care with the patient’s safety as the top priority^^
