Completion on the Same Day After Visiting Through a Dentist’s Referral for Resin Treatment of Peg Lateral Incisors

I never thought I’d end up coming this far~

This is already the fourth dental clinic.

Today’s patient was a woman in her 30s. She had already received consultations at two clinics in Gangbuk.

And after stopping by Kim Jun Yeop Dental Clinic in Apgujeong as the third place, she was told, “It looks like peg lateral resin would work, and Jaejoolgood Dental Clinic is good at resin treatment, so please go there~” That’s how she found our clinic.

Actually, in this case, Director Kim Jun Yeop could have made it look beautiful enough with resin too, so I was really grateful that he even referred her to us. A peg lateral incisor usually refers to the upper second incisor being smaller than the other teeth.

Peg laterals occur more often in women than in men, and according to domestic research, the incidence is reported to be around 1–3%. It’s not common, but it’s not extremely rare either.

I mainly treat resin cases, so I see many patients with peg laterals.

Because of that, patients with peg laterals naturally end up visiting us a lot^^

Actually, the start of the concern was not the “gap.”

What first caught the eye was a slight gap between the two central incisors, but the very first thing the patient said was this.

“This tooth looks too small.”

It was clearly a peg lateral. The patient said the small tooth bothered her more than the gap.

So we first treated the peg lateral with resin.

First, we planned to restore the smaller tooth to a normal proportion.

The problem was that the peg lateral was tilted inward.

And because there was space behind the peg lateral, if we filled that space too, the second incisor could end up looking very large.

So I also explained the option of removing a little from the back side if it became too large.

The patient said it was fine even if there was a gap with the canine.

Also, the upper and lower teeth were biting very deeply.

So I explained that when moving the teeth left and right, there could be a lot of interference with the lower teeth, and that a little adjustment of the lower front teeth might be necessary.

The lower front teeth were not even, and some areas were slightly misaligned, so I thought those parts could get in the way.

So we decided to check this after the resin treatment was completed.

First, we decided to restore the requested peg lateral to a normal proportion and then decide how to handle the space between the front teeth based on that proportion.

Both proportion and shape were aimed at looking natural.

We proceeded without removal, matching only the proportion and curve to the opposite lateral incisor.

At this time, I intentionally chose a design that did not completely fill the space.

Rather than closing the space entirely, leaving a small amount of room and designing the gum line and tooth outline so they connect naturally is better for maintaining gum health and also makes hygiene management easier.

When the patient looked in the mirror and said, “It doesn’t show as much as I thought?” I became convinced that this was the right directionㅎㅎ

Two weeks later, she returned and brought up the central incisor.

“ I’d like to treat the gap between these teeth too.”

We naturally filled the gap between the teeth as well.

I adjusted even the tiny details.

Resin treatment is important, but what happens afterward is also very important.

Even if it looks fine right after treatment, as the gums settle, the resin margins can become noticeable, and there can be subtle discomfort when speaking, smiling, or touching it with the tongue.

So we checked the progress for two weeks after this front tooth treatment as well.

“ It feels like my nail catches a little.” “ When I speak, it feels like my lip is pressing on it a bit.”

Each time, I smoothed it a little and made only the necessary fine adjustments.

Because what matters more than making it perfectly smooth is making it feel natural in my mouth.

I don’t think front tooth treatment is simply a treatment that ends with “looking pretty.”

The impression when smiling, the feeling when speaking, and whether it can be used comfortably over time all need to be considered.

This treatment also did not end with simply making one peg lateral larger; it was a process of carefully identifying the patient’s concern from the beginning and completing it together based on function, proportion, and naturalness.

At first, the patient said, “I never thought I’d end up coming this far,” and I still remember her last look in the mirror with a relaxed smile.

Front tooth resin, especially peg lateral treatment, depends on experience and detail.
