Yeomchang Station Cavity Treatment: I Visited for an Overall Checkup

Teeth do not remain permanently stable

just because they have received cavity treatment once.

Secondary caries, which can occur again

at the boundary or inside the area that was

previously treated,

can appear relatively often, and if it is not detected early,

the restoration area may gradually expand.

In such cases, after removing the damaged tooth structure,

appropriate re-restoration treatment is needed,

and one of the methods used to restore the tooth’s shape and function

is resin buildup.

we will introduce a case in which

This patient had received treatment

at another dental clinic one year ago,

and visited our clinic with the concern that cavities form easily.

They said that because they had previously received cavity treatment

and thought they were prone to cavities,

they came in planning to have an overall oral examination

and cavity treatment.

After checking,

an existing resin restoration was found

in the left lower molar area,

and secondary caries had developed

as bacteria penetrated through a tiny gap

between the restoration and the tooth.

What causes secondary caries?

1️⃣ Tiny gap between the restoration and the tooth (microleakage)

Over time, a very small gap can form

between the resin and the tooth,

and when bacteria and food particles enter through this gap,

a cavity progresses again from the inside.

2️⃣ Aging or fracture of the existing restoration

Because resin is not a permanent material,

if the following occur:

• wear

• deformation

• cracking

• reduced bonding strength

the sealing ability at the margin weakens.

3️⃣ Difficulty maintaining oral hygiene

The area around restorations can, due to its surface structure,

become an environment where plaque adheres easily,

so if brushing is insufficient,

the chance of cavity recurrence increases.

4️⃣ An oral environment with a high risk of cavities

• Reduced saliva production

• Frequent intake of sweets

• Crowded tooth alignment

• A history of multiple cavities

5️⃣ Limitations in the extent of caries removal during initial treatment

If some microscopic carious tissue that is difficult to see with the naked eye

was left behind,

it may progress again over time.

For Yeomchang Station cavity treatment,

after removing the existing resin first,

the cavity was found to be more extensive than it appeared on the surface,

and secondary caries had developed deeply underneath.

In this case, it would have been possible to restore the area

with an inlay, which is a partial prosthetic restoration,

but at our clinic, we first considered a direction that would

reduce the amount of natural tooth removal as much as possible

and preserve the tooth structure.

Accordingly, we planned to restore function and shape

through resin buildup so that the remaining tooth structure

could be stably reinforced.

After thoroughly removing all of the cavities,

although the area was relatively large,

we proceeded with restoration in a way that naturally restored

the tooth’s anatomical shape through resin buildup.

After that, we checked the bite,

carefully adjusted to see whether there were any overly high contact points,

and considered functional stability as well

so that no discomfort would occur during chewing.

By doing the resin buildup without an inlay,

the treatment was completed on the same day.

These are before-and-after treatment photos.

The patient was satisfied because the anatomical shape was better reproduced

than with the previous resin restoration,

and they were glad that the cavity was discovered early through a regular checkup,

saying it was fortunate.

As such, even if cavity treatment is performed,

there is still the possibility of secondary caries afterward,

so regular checkups and consistent oral care remain important after treatment.
