I had orthodontic treatment, but my yellow teeth are still noticeable,
so I want to get laminates.
Many people consider laminates even after orthodontic treatment.
That’s because even if the alignment has improved, the tooth color does not change.
Today, I’d like to introduce a case of a patient who visited because they wanted to further improve their front teeth after orthodontic treatment.
In this post, I’ll explain the following in detail:

Consultation for front-tooth laminates, detailed examination results
The patient visited for a consultation for front-tooth laminates.
(Dr. Lee San-ha handled the treatment.)
They said it had been quite a long time since their last dental treatment,
so we decided to first check the overall condition before doing laminates.
Looking inside the mouth, one front tooth appeared darker from within than the surrounding teeth.
In cases like this, there may be an internal tooth problem,
so we put laminates on hold for the moment and started with an X-ray.
In the photo above, you can see dark inflammation at the root tip.
After examining the X-ray in detail, there were two issues.
First, there was chronic periodontitis overall.
Second, the root area of that front tooth, which appeared dark, had inflammation (a radicular cyst)
that was progressing, so root canal treatment was needed.
The gums and the root must be healthy for the prosthesis placed on top to last a long time.
After explaining the condition in detail,
we decided to proceed in this order: gum treatment → root canal treatment → laminate.
Causes of front-tooth discoloration and how to address them
You can see that the darker color inside is showing through compared with the other tooth in the photo.
If you look at the photo above, the tooth inside the yellow box
looks slightly blackened from within, unlike the others, right?
This is the same tooth in which the root inflammation (a radicular cyst) was found during the examination above.
There are several common causes of front-tooth discoloration,
and they can broadly be divided into external and internal causes.

If only one front tooth becomes noticeably dark,
it is often caused by a problem with the internal nerve of the tooth, as in this case.
When inflammation develops at the root, the internal nerve tissue can
slowly necrose, causing the entire tooth to darken.
So after first resolving the cause of the inflammation with root canal treatment,
the tooth is covered with a crown to improve the discoloration as well.
If I show you the result in advance, you can see that the color difference
from the neighboring teeth is not severe and blends in naturally.

Difference between front-tooth laminates and crowns
“Do I have to get a crown?”
Yes. Since a tooth that has had root canal treatment becomes weaker internally,
a crown that covers and protects the entire tooth is needed.
A laminate is a method in which only part of the tooth surface is thinly reshaped
and a porcelain-like veneer is bonded to it, and it is mainly used on healthy teeth.
Front-tooth laminate and crown placement process
After the root canal treatment was completed, we began the front-tooth laminate and crown work in earnest.
In this patient’s case, as shown below, we applied different prosthetic methods depending on each tooth’s condition.

The existing right canine was a tooth that had already undergone crown treatment after root canal therapy.
The newly root-canaled small front tooth was treated with a ceramic crown,
and the canine was treated with a zirconia crown, using different materials.
Why choose different materials: ceramic crown vs. zirconia crown
The small front tooth is one that is directly visible from the front when smiling.
Because translucency and color reproduction are important to make it look like a natural tooth, we chose ceramic.
On the other hand, the canine is a tooth that receives a lot of lateral force when chewing.
Aesthetic appearance matters, but it also has to be comfortable to use.
Zirconia is also harder than ceramic.

As in the yellow area in the photo above, if there is a metal post inside and the color shows through, that tooth can look darker.
So instead of ceramic, which shows the internal structure more clearly, we used zirconia, which is more stable.
Of course, we needed to adjust the color carefully so that the difference between the two teeth would not be too noticeable.
In this way, choosing different materials according to the position and function of the tooth
affects both the lifespan of the tooth and the aesthetic result.

After finally completing all the settings,
we finished the treatment by attaching a new orthodontic retainer.
