Why do white spots occur?

Why do white spots occur? When the enamel, which is the outer layer of the tooth, does not form properly during development, the tooth surface may become rough, or white or brown spots may appear. In some cases, the tooth shape may also not form properly and may become rough or irregular. When the tooth is forming, light reflects off the less-calcified enamel layer, making it appear opaque, usually white.

They can also occur later in life. If a child under the age of 6, when the permanent teeth are developing inside the gums, ingests too much fluoride, fluorosis may appear in the permanent teeth. They can also occur if a baby tooth is traumatized or develops decay and the tooth bud of the permanent tooth inside the gums is damaged. In addition, after orthodontic treatment, if brushing is not done properly around the brackets, plaque accumulates on the teeth. If plaque is not removed consistently, cavities develop, and white spots can appear due to demineralization, in which the tooth surface is eroded in the early stage of decay.

At Yonsei Jeongwon Dental Clinic, we perform Icon resin infiltration treatment to treat these white stains on the front teeth. Icon resin infiltration treatment is not a method that involves shaving down the existing tooth and covering it with resin. Instead, it is a treatment method that opens the porous structure of the tooth so that the resin can seep in, so there is no need to grind down the tooth. Because it is not a method of bonding resin to the tooth, there is little chance of the resin coming off and a low chance of retreatment, making it a safe and long-lasting treatment.

Recently, a patient visited us because they had white spots on the lower two front teeth since childhood, but more recently they had gradually turned brown, and they were wondering whether it was decay and whether treatment was necessary. In the case of brown spots, this was a more advanced stage from the initial white spots (white spot), and treatment was needed. The left front tooth had a slightly sunken tooth surface along with the brown spot. If the brown spot covers a wide area, it is more effective to first whiten the teeth to remove the darker parts of the brown spot, and then perform Icon treatment about two weeks after whitening.

Because the area of the brown spots was not large, we decided to perform Icon treatment on the two lower front teeth. First, the tooth surface was thoroughly cleaned with a fluoride-free abrasive, and then the tooth surface was sufficiently etched several times with 15% hydrochloric acid to create a porous structure. After etching, the area was sufficiently dried, Icon resin was applied and polymerized, and then the tooth surface was smoothed and polished. In this case, fortunately, the area of the brown spot was not large, so the brown spot disappeared with etching alone.

This is a photo taken immediately after Icon treatment. You can see that the white spots and brown spots improved with Icon treatment alone, without any additional resin treatment. Since the photos are taken with the teeth dried, if you look closely, it may seem as though the white spots have not completely disappeared. However, in the actual mouth with saliva present, they look much more aesthetically natural than in the photos, and after about 1 to 2 weeks, they appear much more natural.

If the demineralization is extensive and deep, the mottled tooth may not be completely removed, and depending on the condition inside the tooth, if the porous structure of the tooth does not open fully, Icon resin infiltration may be difficult.
