In my previous post, I talked about cases in which, when the dental midline was deviated, treatment was performed without extraction, or by extracting one small premolar from the deviated area, resulting in non-extraction or minimal-extraction orthodontic treatment.

This patient began orthodontic treatment with the chief complaint that the lower jaw was protruded, causing the front teeth to bite in reverse, and that both the dental midline and the lower jaw were deviated to the right.

Because the lower jaw was deviated to the right (left side in the photo), there was also a significant difference in the canine occlusion on the left and right. To improve the asymmetry of the dental midline and the protrusive appearance, one upper left premolar and both lower premolars, a total of three premolars, were extracted.

After completion of the orthodontic treatment, the dental midlines were aligned, and the uneven canine occlusion on the left and right also improved.

Looking at the frontal facial photo before treatment, the lower lip appeared more protrusive than the upper lip, and it was clearly visible that the lower teeth were more prominent when smiling. After treatment, the fullness of the upper lip became more apparent, and the patient was able to smile more naturally. As the extraction spaces in the lower teeth closed, the protrusion of the lower lip was also significantly improved in profile view.

The patient below visited the clinic with the chief complaint that the jaw was deviated and the mouth protruded. The midline of the upper teeth and the facial midline were aligned, but the midline of the lower teeth was deviated to the left (right side in the photo), and the left front teeth and canine area were biting in reverse.

To resolve the protrusive appearance and improve the dental midline, four premolars were extracted, and the spaces were used to align the midline and improve the protruding mouth.

After completion of orthodontic treatment, the dental midlines were aligned, and the reverse bite in the molars also improved.

Skeletal asymmetry of the upper and lower jaws can only be treated with orthognathic surgery, but asymmetry caused by differences in the upper and lower dental midlines can show excellent improvement with orthodontic treatment alone. Therefore, in cases of asymmetry, I recommend first receiving an accurate diagnosis to find out what is causing the asymmetry. :)
